A $50 reduction in team fees will apply to men’s teams entered in MDIII if they have another team entered in MDI, MDIAA,
MDIAAA or MDII, as well as

Women’s DI, DIAA, or DII.
Entry fees are non-refundable. (Except for teams on a waiting list that don’t get into tournament).

ENTRY APPLICATION PROCEDURE
The entry application represents a team's tournament entry which has been completed by the Team
Representative through the NCVF Online Registration System.
Use the NCVF Tournament Registration System – www.vbreg.com :
Deadline to have all steps of Tournament Registration Completed is Feb 20, 2015. Spots are filled as registrations are
completed so make sure to get your entry completed ASAP. Just because it is in by the deadline does not mean that
there will be spots available in your requested division.

 Complete/Update all team and player information on the NCVF regular season roster from the team rep
menu before registering for the tournament.
Tournament Registration –
1. Enter your team into the event once you have completed all the team entry data (player and team
information) by selecting the “National Registration” link and selecting the team you want to
register.
2. Complete the required team entry information and accept all conditions.
3. If you only have one team to enter, select the “Create Invoice” button. This will calculate the entry
fee based on the memberships you have completed.
Entry Fee Payment - Complete the payment - Payment Methods:
1. PREFERRED - Credit Card Payments can be made online by clicking the Make Payment link under the
Invoice Number (www.vbreg.com). There is a $27 processing fee.
2. Checks are accepted but spots fill as payments are received. We have had issues with checks from
universities being much delayed and teams missing deadlines. Make checks payable to: USA
Volleyball. On the Memo line of your check, PRINT the team name and division. Staple the check to
the entry form/invoice printed out from the website.
3. Ship via UPS or FedEx and require tracking on the package, the completed Entry Form/Invoice with
attached check to:

1. Select “Roster & Forms” link next to you’re the team you just registered in order to complete your
Tournament Roster.
2. Player are chosen from the available players on your NCVF Season Roster and added to the
tournament roster. If you do not have players on your regular season roster, there will be no
players to add to the tournament roster.
3. Teams are limited to 15 players at the national tournament. Players cannot be added to the
Tournament Roster after the Feb 20 roster deadline. Make sure you have all players on the roster
by that date including any potential players that will be attending in case of injury to primary
players.
Required Forms
1. All participants will be required to complete waivers, USAV membership form, and code of conduct
forms. We are working to get these forms so they can be completed online. More information will
be sent out to team reps who have registered in January.
Housing Reservations through THS - Login to the THS Housing system and make room reservations at a
tournament hotel using the team name and division listed on your entry form. See Book Hotel tab.

All individuals listed on the roster are required to be registered as members of USAV by completing the Limited
Membership Form provided. Any coaches, assistant coaches, or team reps travelling and working with the team
must be added as team representatives during registration and complete the Limited Membership Form.

NOTE: Your entry is not considered complete, and will not be processed until the following is met:
1. Proper Entry Fee received.
2. Housing reservations completed through THS

must be completed by their established deadline

Failure to follow these procedures will jeopardize or delay the acceptance of your entry application. No
team will be granted entry into the championships until USA Volleyball has received all COMPLETED
required forms and signed by the appropriate authority. Incomplete forms will not be accepted. Do not fax
entry application forms to USA Volleyball.

Rosters must be completed and submitted online. Reps must complete the tournament roster
by transferring players from the NCVF season roster to the TOURNAMENT ROSTER. NO
ROSTER ADDITIONS WILL BE ALLOWED AFTER THIS DATE. Any players that may be
considered to compete in the tournament must be on the roster by this date.

verification forms are not presented at the on-site registration area, that team will not be allowed
to participate.

Notes

Tournament Entries will be accepted beginning January 3, 2015 with the entry deadline at midnight on February
20, 2015. Please note that many divisions fill prior to the entry deadline and by having a complete entry in by the
entry deadline does not guarantee entry. All entries will be marked with an “Entry Complete Date” that will be used

to determine the order of accepted teams as division reach capacity. The sooner a completed entry is received,
the better a team’s chances of being accepted into the championships in their preferred division.

Enrollment Verification Forms: must be brought in person to the on-site check-in on Wednesday, April 8 at the
convention center verifying each participating student’s academic status of the institution they are representing.
 Semester Schools: Students from schools that have academic semesters starting during the month of
January and continuing through the championships date must be enrolled 3/4 of full-time enrollment as an
undergraduate or 6 or more credit hours as a classified graduate student.
 Quarter Schools: Students from schools that have academic quarters must be enrolled 3/4 of full-time
enrollment as an undergraduate or six (6) or more credit hours as a classified graduate student during
both winter and spring quarters.

Post Event Enrollment Verification: The top four teams in each division and five randomly selected teams from
each division will be required to fill out and submit a second enrollment verification form following the tournament.
This second form must be filled out, signed and stamped by the registrar, and submitted to the NCVF National
Center between April 10 and May 30. The top four teams/division and those randomly selected must fulfill this
requirement. Those teams that fail to submit the second certification form, or have students that are not enrolled in
the required number of credits will not be allowed to enter the 2016 NCVF National Collegiate Club Volleyball
Championships and will potentially be disqualified from their final finish.

A school photo ID will be required to establish identify of players prior to each match at the court during competition.

VERIFICATION OF ENTRY IN THE TOURNAMENT

Teams will be able to check the progress of their team entry online. Entry status will be available online.

ALL teams that submit complete and accurate materials will receive an e-mail notification.

Teams may check the tournament registration website, www.vbreg.com to verify their entry.
REFUND POLICY
Any team that is denied entry into the NCVF Collegiate Club Volleyball Championships will receive a full refund.
NO ENTRY FEE, OR ANY PORTION OF SUCH FEE, will be refunded to a team that withdraws from the tournament AFTER the
entry fee payment is received.
USAV shall issue NO REFUNDS when an individual or a team decides not to attend, or is unable to attend the
tournament for reasons pertaining to acts or threats of war, acts or threats of terrorism or a health risk alert.
When the tournament is canceled or interrupted due to acts or threats of war, acts or threats of terrorism, or a health
risk alert, event entry refunds will be made to individuals or teams on a prorated basis once all out-of-pocket
expenditures for the canceled or interrupted event have been settled from such fees.
ENTRY LIMITATION
It may be necessary to limit the number of entries into one or more of the tournament divisions because of the
anticipated number of entries into the tournament. The Championship Committee reserves the right to establish an
entry limitation in the best interests of the tournament, the Event and participating teams once the guarantee has been
satisfied (see below).

The guaranteed maximum number of teams that will be accepted into the tournament is 48 teams per
division.
GUARANTEED ENTRIES
Teams that are ranked in the Top 25 of the Men’s or Women’s Poll previous to the tournament entry deadline will be
given priority entry into the appropriate division classification provided their entry packet is completed by the published
deadlines. Automatic entry will not be guaranteed for teams that do not have a completed entry by all published entry
deadlines.

TOURNAMENT TEAM CHECK-IN
Team representatives and/or coaches whose teams participate in this Event are required to check in on April 8, 2015
no later than 8:00 PM. All check-ins will be in the Convention Center. The check in process will include the following.
TEAM CHECK-IN
Registration/Check In - All teams MUST check between 4:00pm - 8:00pm at the convention center. The
team rep/coach must check in for each team. Please plan your travel arrangements accordingly to attend
the meeting.
Roster Verification - Team reps will review the electronic roster and complete a FINAL confirmation of
names and jersey number for all attending players. Any roster changes required at this time will incur a $50
change fee.
Uniform Verification - Team reps must verify that their team jerseys/shorts are legal at the uniform
verification station.

Uniform numbers must be a recommended 6” with minimum of 4" in height on the chest and a
recommended 8” with minimum of 6" in height on the back.

Only numbers 1-99 are legal.

All sets of jerseys (light and dark) must have the same number on them. No second numbers will be
allowed for any players. If a player is playing as a libero, their libero number and regular jersey
number must be the same. However libero jersey must a contrasting color top.

Jersey numbers must be in sharp contrast to the jersey’s main body color.

If playing with a designated Libero, that player needs to have their own jersey number as well as a
different color jersey top.

Team reps do not need to bring each player’s jersey; instead, the team rep should bring an example
of one jersey per color.
Onsite Tournament Contact Information – Complete contact information form for onsite communication
during the event for team reps and liaisons.
Mandatory Team Rep & Coaches meeting will be held at 8:15 pm in the convention center. The coaches
meeting will last approximately one hour. A coach or team representative must attend. Please plan your
travel arrangements accordingly to attend the meeting.
LATE TEAM CHECK-IN
Teams checking in after 8:00 PM will be charged a late fee of $50.

CAPTAINS’ / TEAM REPRESENTATIVE MEETINGS
All Captains / Team Representatives are required to attend a mandatory Captain’s Meeting in the Convention Center
at 8:15pm. Playing rules, tournament schedules, tournament policies and procedures will be reviewed at this time.
The meeting will last approximately one hour
Team Liaison: Each participating college/university will be required to appoint an Official University-Approved Team
Liaison to accompany their team(s) at the NCVF National Collegiate Club Volleyball Championships. The Team
Liaison may be a player and must be authorized by the college/university Recreational Sports Director, and will be
responsible for their team’s behavior while at the NCVF volleyball championships. The Team Liaison will be
responsible for ensuring that their team(s) will follow the Standards of Conduct. Team Liaisons are required to attend
the mandatory coaches/team rep meeting.
