2015 Coed Ice Hockey
Player Evaluation Form
Coed Ice Hockey is designed to mix up players of all abilities for three days of hockey fun.
Women may be 45 years of age by December 31, 2015. You must return this form to AISG
by July 25th. We are expecting enough registrations to fill four Teams of at least 11 players.
Players will be selected on a first come first serve basis. Each team will play a minimum of 3
games on August 8th, 9th, and 10th.
All Teams will play in the same age group. Players may score only one goal per game. No
checking. No slap shots. Teams must have a minimum of 2 men and 2 women on the ice at
any time, not counting the goalie. Commissioner will form teams and set game times.
