Westside Fall Instructional League
Hello and Welcome!
Thank you so much for wanting to be a part of our very first youth softball league this fall.
We are hoping that this league will help you and your players evolve into the team that you are
hoping for with an increased skill base for your players and an increased knowledge base for your
coaches.
In order to register your player for this league, we ask you to complete the next couple
pages and then return these forms as well as any fees associated with playing to The BASE, located
at 6801 Commerce Ave, Bldg. 7B in El Paso. If you have any questions, please feel free to call The

Player Waiver
In consideration of being allowed to participate in the Chihuahuas NeXt 8U & 10UFall League, or other activity organized
by El Paso Border Youth Athletic Association, hereafter referred to as “EPBYAA”, the undersigned prospective player
(“Player”) agrees to assume all risks incidental to such participation (including, without limitation, injury or loss to
person or property).
Each of the undersigned hereby agrees to release and forever discharge individuals from EPBYAA from any and all
liabilities, claims, actions, damages, costs or expenses of any nature whatsoever, whether in law or equity, known or
unknown, that any of the undersigned ever had, now has or hereafter can, shall or may have against any of THE BASE
arising out of or in any way related, directly or indirectly, to Player’s participation in such tryout, workout, or other
activity.
Each of the undersigned parent(s) or guardian(s) of the Player hereby agrees to indemnify and hold harmless each of the
individuals from and against any and all liabilities, claims, actions, damages, costs, or expenses of any nature
whatsoever, whether in law or equity, known or unknown, incurred by any of the individuals and arising out of or in any
way related, directly or indirectly, to Player’s participation in such tryout, workouts, photographs or other activities.
Each of the undersigned parent(s) or guardian(s) of the Player hereby ratifies the Player’s execution of this waiver and
release form.
Parent, Player, Player’s parent(s) or guardian(s) hereby authorize El Paso Border Youth Athletic Association,
hereafter referred to as “EPBYAA,” to publish photographs taken of Parent/Player, and Parent’s/Player’s name
and likeness, for use in EPBYAA’s print, online, social media and video-based marketing materials, as well as,
EPBYAA publications. Each of the undersigned agrees to release and hold harmless EPBYAA from any
reasonable expectation of privacy or confidentiality associated with the images specified above. All undersigned
acknowledge that Parent’s and/or Player’s participation is voluntary and that there is no financial compensation of
any type associated with the taking or publication of photographs, company marketing materials, or any other
EPBYAA publication.
**If Player is UNDER THE AGE OF 18, at least one parent or guardian MUST COMPLETE and SIGN this form
in order for Player to participate in any activity by EPBYAA.**
