GP / Community Referral Process Paul Carnduff GP Referral Process • GP makes decision to refer – Appropriate for service – Selects Referral Wizard which is pre-configured with services and methods E-referral - GP • The e-referral screen is displayed • Sharing preferences if not previously set will display E-referral - GPl • If patient has already expressed consent no message will display • For records where the patient has declined consent will display message E-referral - GP Pre-Configured referral details with option to select Reason if required for those services as needed Sharing in at Receiving Organisation Explicit consent for viewing the record at the service being referred to is obtained at the point of referral by the GP E-referral - GP • Protocol used if additional information required e.g. SAT Form for Mental Health – Checks the Sharing preferences and prompts user – If required points to referral form • Option to E-mail additional information if required (non-shared records) – Recording within the patient record • Discharge summaries received electronically and processed through the document workflow E-referral - GP • Tasks received for each stage of the referral process E-Referral - Community E-Referral - Community • Retrieving a record before accepting referral Explicit sharing consent obtained by GP at point of referral E-Referral - Community E-Referral - Community • Patient Registered at service when received – referral in details to complete • Patient can be added to caseloads – Tracked and status updated – No referrals lost – RTT • Protocols and Templates • Full Audit trail and Reportable • Tasks – Able to task the practice or receive tasks from the practice • Discharges – Information sent to GP electronically via SystmOne
