One male and one female athlete will be chosen to receive a $3,500
scholarship. All applications packages need to be turned in to the HHS
Counseling Center on or before April 25th, 2014.
In addition to this completed application form, please provide the following
supplemental information as part of your application package.
 Two letters of recommendation.
o One from a coach
o One from a teacher, advisor, or community member
 Official copy of high school transcripts
Note: Panel interview may be scheduled with some applicants.
