The following documents must be presented at the time of registration:

Child’s Complete Immunization Record (for more information please check FISD
website)

Child’s Certified Birth Certificate

Child’s Social Security Card

Proof of Residency
If living in own residence or renting
Provide current utility bill showing name, current address and date
Provide rental agreement or lease (if renting or leasing)
If living in another person’s residence
Provide photo identification of person responsible for (Owner of) residence (OWNER MUST
BE PRESENT)
Provide utility bill showing name of owner, current address and date
Residence owner to complete affidavit of residency & notarized (the form and notary will
be available during registration)

Parent/Guardian Photo Identification

Current Proof of Income (2 most current paycheck stubs and Verification of
(SNAP/TANF) or current Medicaid Card or Active Military documentation)
Head Start Documentation for proof of income MUST include:

Current W-2, Form 1099, written letter from employer, bank statement, TANF, or SSI

Child Support payments must be reported as income
Children must be either 3 or 4 years old on or before September 1st of 2017 to be eligible for the PK 3
(Head Start eligible ONLY) or PK 4 programs.
*Presenting false information or false records for identification is a criminal offense under Penal Code
37.10 (FD legal).
