The Turtle Bay Junior Golf Academy develops
the fundamentals of golf in a fun and safe
atmosphere. Our goal is to help juniors learn to
play better golf while also establishing a lifetime
love for the game. Participants in the Turtle Bay
Junior Golf Academy earn additional privileges
to practice on the range, play the golf course and
compete in local tournaments.

I agree that in the event that my child exhibits signs of illness or
injury and if the Turtle Bay Golf staff is unable to contact me or
any emergency contact listed above, my child may be taken to the
nearest medical facility and given any treatment deemed necessary
by the personnel of the medical facility. I understand that injuries
can arise by accident from the very nature of golfing activities. I
hereby release and waive all rights to any claim or action against
Turtle Bay Golf arising from injury, loss or damage to my child and/
or my child’s property except where such injury, loss or damage
is caused by willful misconduct or gross negligence of Turtle Bay
Golf. I authorize Turtle Bay Golf to use my child’s name, video or
photograph at any time and in any manner in connection with its
advertising, publicity and public relations programs. I agree that a
parent or registered guardian must be present at the end of each
class to pick up my child. No child will be released off property on
their own or with another minor.
