Section I: In this section complete the tournament information you are attending and the team information for
the team that is attending the tournament.
Section II: In this section complete the coach/player information that will be attending the tournament. Please
remember that each coach/player must be a current ISA member.
This should be submitted to the ISA office at least 2 weeks prior to the tournament.

Tournament Teams - A team composed of players from more than one registered team. This team can only enter
tournaments and competitions that allow Tournament Teams. Tournament Teams at the U10 age division and below
are prohibited.

E. Youth players may change teams during the seasonal year in the following manner:

Member Organizations may form Tournament teams in U11 through U19 age divisions from players who are
rostered with Member Organization teams. These teams are registered for one event only and will be so
designated on the roster. The Association will charge a fee for each Tournament roster. The roster must be
completed and submitted by the Member Organization from which the Head Coach of the team is registered.
