We wish to take up the free 3 week trial period with Burghfield Football Club.We shall abide by the Conditions
of Membership and Codes of Conduct set out below and shall adhere to them at all times. The Player Details
and Contact Information provided are correct and I shall advise the Club of any changes as and when they

CONDITIONS OF MEMBERSHIP
1. Save for those circumstances where exemption/limitation of liability is not permitted by law, Burghfield FC or its
officials will not be liable for any illness or injury arising directly or indirectly as a result of training or before, during
or after a match or training session (including transportation to and from such matches or training sessions)
however caused.
2. Burghfield FC or its officials will not be held liable for any personal items lost or damaged in any way whatsoever.
3. Parents/Carers are responsible for ensuring that players attend training sessions and matches properly and
adequately dressed to take part in sporting activities, as required by the Laws of Association Football (including
the wearing of shin pads), and that such items are at all times in a satisfactory condition.
4. Such subscriptions and match fees as may from time to time be set by the Club Committee to defray the
expenses incurred by the Club must be paid when requested.
5. My child and I have read and understand the Codes of Conduct attached. We agree to be bound by it and accept
that the Club will impose sanctions against those people who fall short of the standards of behaviour required by
the Club. A copy of the Codes of Conduct, including sanctions, is on the website under ‘Respect’.
6. Completion of this form does not guarantee that my child will be picked to play in any or all matches played in their
age group team.
7. Suitable photographs taken at training or matches may be used in club newsletters or on the club website. When
used on the website, names will not appear in association with the photograph.

CODES OF CONDUCT
Young Players
We all have a responsibility to promote high standards of behaviour in the game. As a player, you have a big part to
play. That’s why The FA is asking every player to follow a Respect Code of Conduct.
When playing football, I will:







Always play to the best of my ability
Play fairly – I won’t cheat, complain or waste time
Respect my team-mates, the other team, the referee or my coach/manager
Play by the rules, as directed by the referee
Shake hands with the other team and referee at the end of the game
Listen and respond to what my coach/team manager tells me
Talk to someone I trust or the club Welfare Officer if I’m unhappy about anything at my club.
Spectators and Parents/Carers
We all have a responsibility to promote high standards of behaviour in the game. This club is supporting The FA’s
Respect programme to ensure football can be enjoyed in a safe, positive environment.
Remember children’s football is a time for them to develop their technical, physical, tactical and social skills. Winning
isn’t everything.
Play your part and observe The FA’s Respect Code of Conduct for spectators and parents/carers at all times.

Remember that children play for FUN
Applaud effort and good play as well as success
Always respect the match officials’ decisions
Remain outside the field of play and within the Designated Spectators’ Area (where provided)
Let the coach do their job and not confuse the players by telling them what to do
Encourage the players to respect the opposition, referee and match officials
Avoid criticising a player for making a mistake – mistakes are part of learning
Never engage in, or tolerate, offensive, insulting, or abusive language or behaviour.
I understand that breaches of the code may result in action being taken by Burghfield Football Club, the
County FA and /or The FA. A copy of the Codes of Conduct is on the BurghfieldFC.com website under ‘Respect’.
