Instructions
1. Complete your venue details.
2. Complete the details in the Game Details box.
3. For a single game complete the details in the Game 1 parameters box or for a multi game, complete
the game parameter details in the Game 1 to Game 8 boxes depending on the number of games (2 to
8).
4. Attach a copy of the VCGLR Approval Letter, Central Database Submission Form and the Game Image
file.
5. When you have completed the form, send it to Intralot at [email protected] or fax to
03 9673 3999. Arrange for the Game chipset to be sent to Intralot.
