2015-16 Proof of U. S. Citizenship Status
INSTRUCTIONS
Students who are required to confirm their U.S. citizen status must either:

Call the Consultation Line at 866-705-0200 to schedule an appointment to submit the required documents.
At the appointment, the student will appear, in-person, at the Student Financial Services office with an
original document (see list below) and complete Section A in the presence of an SFS Representative; or

In the presence of a notary, complete Section B and present an original document (see list below) to the
notary; a copy of the documentation submitted to the notary along with the completed Section B must be
returned to Student Financial Service via postal mail (fax, email, and electronic document submission are not
permit table) to the address: Student Financial Services, 600 S Michigan Ave Suite 601, Chicago, IL 60605.
Students must present a valid, government issued photo ID along with one of the following acceptable documents as
proof of U.S. citizenship status:

A Birth certificate showing s/he was born in the United States *; or

A signed U.S. passport, (current or expired); or

A valid Passport Card (current or expired); or

A signed N-560 or N-561 (Certificate of Citizenship) issued by USCSIC; or

A signed N-550 or N-570 (Certificate of Naturalization); or

Form FS-20 (Consular Report of Birth Abroad); or

Form FS-545 (Certificate of Birth Issued by a Foreign Service Post); or

DS-1350 (Certificate of Report of Birth)
* Including Puerto Rico on or after 1/13/1941; Guam or the Virgin Islands on or after 1/17/1917. Citizens of the
Residents of American Samoa, Swains Island or the Northern Mariana Islands are considered U.S. citizens as long as
their parent(s) were not foreign diplomats.
All documents presented in-person will be photocopied (front and back) by an SFS Official. Students completing
this form via notary must submit copies of the front and back of all documentation.
Confirmation of Eligibility
Student Financial Services will review the documentation submitted, upon receipt of this form, against the
information reported on the institutional record of the student’s Free Application for Federal Student Aid (FAFSA).
SFS is unable to provide estimated financial aid awards for students until this process is complete.
