WAIVER 2016/ 2017
Assumption of Risk: I hereby certify that my child/children is/are in good physical condition and able to
participate fully in all UEC/CDM program or class for which she/he/they is/are registered.
Release of Liability: I hereby release UEC/CDM and its teachers from liability in case of accident or injury
howsoever caused. UEC/CDM is not responsible for any loss of or damage to personal property.
Medical Emergencies: All current medical conditions and those requiring medication have been disclosed on the
registration form. I understand that in the event that of a medical emergency, a representative of UEC/CDM will
notify me as soon as possible at the phone numbers I have provided. In the event that I cannot be contacted to
provide consent, I hereby authorize a representative of UEC/CDM to give consent for any medical treatment that
may be required for my child. I understand the representative of UEC/CDM will use her best judgment in obtaining
such service for our child and that I will be responsible for any reasonably related expense.
Media Release: UEC/CDM (and the competition companies attended by our family for *competitive program
members) has my permission to use photos and video of my child/children, taken at recitals, performances and
competitions, on the school's website at www.cdmdance.com and for any promotional material and social media
websites (such as YouTube, Facebook & Instagram) related to UEC/CDM. I understand that these images will not
be given or sold to any outside individual or organization.
Payment Policies: Tuition for the 2016/2017 season must be PAID IN FULL, by POST-DATED CHEQUES or by
PRE-AUTHORISED PAYMENTS in three installments dated September 12th, 2016, December 5th, 2016 and
March 20th, 2017 (students age 6 & under may pay first term only if preferred). UEC/CDM will charge a $40 nonnd
refundable administration fee per family (unless waived with pre-registration before July 2 , 2016) which is
payable with Term 1 payment due September 12th, 2016. UEC/CDM will charge a $50 recital costume deposit
per dance participating in recital, which is payable with Term 1 Payment at time of enrollment. This charge is

NON-refundable after DECEMBER 5 2016, confirmation of participation is also due on this date.
Late registration: A late registration fee of $10.00 in addition to the $40 registration fee will apply if I register after
the first day of my child/ren’s class.
Missed Classes / Cancellations:
Please let us know if you will not be able to attend a class (via email or telephone). We will pass this information
on to their teacher. Missed classes must be made up within the same term of your absence. No credit will be
given for missed classes. You can schedule your make-up class online through your own online account. No
credit is given for student absences. Students who observe religious or cultural holidays not noted on our
calendar that require a student to miss a class can choose a suitable make-up class with the studio manager or
director. No credit will be given on the account. CDM reserves the right to cancel any class with a low enrolment
of less than 4 students or reduce the duration of the class on any given day if fewer than 4 students attend. We
will give notice of a cancelled class and will do our best to offer an alternate option. Classes cancelled due to
unexpected inclement weather will not be rescheduled outside of class time. However, classes can be made up in
any existing class at or below the students level. Make up classes will be made up if a teacher is unexpectedly ill
and a suitable substitute could not be found in time, or for any other unplanned cancellation (Carte Blanche
classes and extra competitive rehearsals are the exception).
Late Payments & NSF Fees:
Late Payment Fees: Outstanding payments are subject to a 2% late charge after 14 days from the initial
payment due date. After 30 days from the initial payment due date, another 2% will be charged. After 45 days,
CDM reserves the right to prohibit students with outstanding fees from attending classes until full payment is
received. Course of action may be taken including possible termination of enrolment.
The quality of our facility and programs depends upon a system of mutual respect and cooperation. We
understand that anyone may make a late payment from time to time, but please be respectful of our late fee policy
and when applicable, graciously include your late fee in your next payment.
NSF cheques are subject to a $25 charge.
If your credit card information has changed, please notify the office before the next payment deadline.
Withdrawal Policies / Refunds: We have a very fair policy regarding our payment for tuition and subsequent
assessment of late fees. Any student who withdraws from a class will be charged a $50 withdrawal fee. Any
student who withdraws before October 23, 2016 will be refunded a pro-rated amount of the Term 1 Payment paid
in September. Unprocessed post-dated cheques for remaining terms will be returned in full and any future preauthorised payments will be cancelled. Any student who withdraws from a class on or after October 24th, 2016
and on or before December 4th, 2016 will be refunded payments for terms 2 and 3 only.
No tuition refunds will be issued after April 23rd, 2017. Absences do not confirm that a student has withdrawn. All
withdrawals must be submitted via email. This email will determine official withdrawal date.
Competition Entry Fees Refund Policy- After December 7th, 2016 refunds will not be granted. CDM will not refund
for injuries occurring due to an extra-curricular activity. Credit may be given towards the competition company we
attend if applicable. However, each case will be taken individually.
MP: Please note that although the current schedule is current, class times, dates, teachers and studio allocation
ARE SUBJECT TO CHANGE over the summer to accommodate low enrolment, staffing scheduling and
other factors. Any withdrawals that families must make due to a schedule change that does not work for your
family, we will suggest an alternate class for you.

The Fall Dance Season is divided into ‘Terms’. Officially the dates are as follows. Note that 3 weeks of holidays
are within these dates:

Term 2: December 5th, 2016 - March 19th, 2017
Term 3: March 20th - June 11th, 2017
