U4(Little Kickers): birthdays between 8-1-11 and 4-09-13

Recreational League soccer is PCYSA’s intramural soccer division. It consists of teams from U4 (Little Kickers) to U14 (Under-4 to
Under-14). Players will be placed on teams based on their ages. Rec League soccer is an instructional league with emphasis on fun

**New This Season, Please Read** Required Equipment for Play: All players will be required to have cleats/tennis shoes, shin
guards, and **his/her own soccer ball** for each practices and games (see age group listing above for ball size). Coaches will only
be provided with three or four soccer balls, the only way to ensure that your player has a ball to use is if you bring your own. This
will also help with the development of your player!
Team assignments: Siblings will be assigned to same team IF they are at the same level. Any parent who coaches will be placed with
his or her child. No other special request (relating to friends, transportation, favorite coach, etc.) will be considered.
The first 2016 Spring Season soccer game will be held on April 9 , 2016. The season will be played on 7 weekends (7 games for
U4/U6/U8; 6 games and a jamboree for U10/U14). All games will be held on Saturday mornings at the Big Walnut Sports Park. The
U4 age group, Little Kickers, will ONLY play on Saturdays. They will not have practices during the week. All other teams will
practice once a week beginning the week of March 28 as weather allows. See Important Dates on the back of this sheet for more info.

Community Based Teams: If you do NOT live in Greencastle and would like your child to be considered for a community-based team (numbers
permitting) that MAY practice near your home but will still play its games in Greencastle, check the community nearest your home below (the only
guaranteed practice facilities are at Big Walnut Sports Park):

Medical Release (your child may not play until this is signed): I hereby acknowledge that my child may be injured by participating
in this sport. I am aware that there is no Benefit Fund for participants in this program, and I agree to assume personal responsibility
for any injuries that my child may suffer. I do hereby release and discharge PCYSA and their respective agents, officers, coaches and
all volunteers, Big Walnut Sports Park, City of Greencastle, and Greencastle Community Schools from all claims, demands, or
damages due to any injury. In the event of a medical emergency, I hereby give my consent for a PCYSA representative to obtain
medical treatment for the player named on this form.

PCYSA Code of Conduct
I/we, the parent(s)/guardian(s) of the named player, also agree to the following:
1. To abide by the squad / level placement made by PCYSA coaches, and understand the balance that must be struck between
the needs of individual players, and the need to form viable, appropriately balanced and competitive squads.
2. To understand that PCYSA is a youth development program, and it is about the players: making friends, playing soccer, and
gaining skills and confidence. Making mistakes is part of the game, and important to learning.
3. To let the coaches do the coaching.
4. To respect everyone involved in the game, which means: not talking to/yelling at opposing players, referees, or coaches, and
respecting parents of the opposing team. To treat those who visit our club facility as welcome guests. In short, to model good
character and support good values for all members of the club.
5. To address concerns or problems constructively to coaches or board members, who can forward reports to the league.
