Notification of Child Missing from Education / Removed from School Roll
This notification should only be completed for the following reasons:
1. A child has failed to attend school, no reason has been given for absence
and a MASH referral has been completed due to concerns that the child is
at risk of significant harm.
2. A child has failed to attend school for 10 days or more and no reason has
been given for the absence.
3. The parent has elected to educate their child at home;
4. The child no longer resides within a reasonable distance of the school;
5. A child has been certified by the school’s medical officer as not being in a fit
state of health to attend school before ceasing to be of school age;
6. The child has been remanded into custody for a period of four months or more
and the child will not be returning to the school upon release.
7. The child has been permanently excluded

Schools/Academies will be informed by the Local Authority once the child may be
