Acceptance Letter
Congratulations on your entry into the 2014 Kohl’s Nebraska American Cup! Please help us make this the best
Kohl’s Nebraska American Cup ever by keeping the spirit of the game in mind during the competition.
Team Check-in (no players):
Team Check-In for Omaha-metro teams will be held on Wednesday May 14th from 5:00pm - 7:00pm at
the Nebraska State Soccer Office (10700 Sapp Brothers Drive – Suite B, Omaha, NE 68138). Teams from
outside the Omaha-metro may check in one hour before your first game at the Tournament Tent! If you are an
Omaha-metro team and cannot check-in on Wed., 5/14, please contact Tessa Bowen at (402) 596-1616, X102
to make schedule an appointment. Players are not required to be present – only the coach or team
representative. Coaches must remember that there may be problems with required forms for check-in causing
problems for some players. Coaches need to make sure they are prepared and if you have any questions
to contact the Nebraska State Soccer Office at (800) 909-4458 or (402) 596-1616 prior to the
tournament.
The following documents (explained below) will be required at team check-in:
1. US Youth Soccer Pass for each coach and player (with photos attached & laminated). Players need
not check-in individually.
2. Medical Release Form for each player, signed by the parents/guardians.
3. The Tournament Staff will have your Certified Roster and Guest Player Form (if needed). You cannot
add players to the roster at the team check-in.
4. Alliance 4 All Player and Parents Form – one for each player and their parents/guardians.
5. Alliance 4 All Coach Form – one for each coach listed on the roster.
Alphabetizing your player passes, medical releases and Alliance 4 All forms will help to expedite
your check-in.
T-shirts: We will have a limited supply of tournament t-shirts on sale at Tournament Headquarters for a price
of $15.00.
Awards: All participants for the U09 and U10 divisions will receive medals. U11 and older age groups will only
receive first and second place medals. PLEASE BRING YOUR TEAMS (ALL PLAYERS) AFTER YOUR
LAST GAME TO THE TOURNAMENT HEADQUARTERS TO RECEIVE YOUR MEDALS.
Important Information on Secondary and Guest Players: Remember, Secondary and Guest players must
come from a team playing in ENSA, CYSL or an in-house league. The forms will be checked for compliance.
You must have a Guest Player Form showing each player. You are allowed three (3) Guest players. Make sure
you follow the recruiting rules by contacting the Primary Coach of the player you are asking to play with you,
prior to contacting the player. Guest Player Forms must be submitted on www.thetournamentcenter.com by
May 13th. This will give the Nebraska State Soccer Association Staff time to check players to make sure they
qualify.
Important Information on Rosters: Nebraska State Soccer Staff will review your current certified roster for
ineligible players. If any players on your roster are crossed off, they are ineligible to play in the tournament.
The Tournament Staff will have a copy of your Certified Roster and Guest Players Form at check-in for your
team. If you make changes your roster between now and check-in, please notify Tessa Bowen at (402) 5961616 x 102 or [email protected].
US Youth Soccer Pass: All coaches and players participating must carry a current year US Youth Soccer
pass. Passes must have a picture and be laminated. If the coach uses illegal players, coaches will be
suspended or fined or both. All teams are also required to carry written Medical Releases signed by the
parents/guardians for each player in case emergency medical care is necessary. This form is available on the
website at http://www.nebraskastatesoccer.org/stateadmin/forms_and_documents/ Medical Releases are not
required to be notarized; this is a policy of US Youth Soccer and Nebraska State Soccer Association.
Alliance 4 All
Each player, parent, coach and referee will be responsible for viewing a video, reading a code of conduct,
signing the Alliance 4 All Form, and then submitting the form. Players, parents and assistant coaches submit
the form to the coach. The coach will show the forms during check-in. Referee submit the form to the Referee
Assignor. There is one form for each player and their parents/guardians, and a separate form for each coach

Accommodations, Transportation and Expenses: Your Entry Fee includes cost involved in putting on the
tournament. You will be responsible for transportation, lodging, meals and any other costs associated with the
tournament.
Referees: We will be using USSF Registered Referees on all games with a three-person crew on U11 and
older age groups games. We can never have too many referees. Referees are invited to officiate games
during the tournament. Individuals interested in officiating games should go out to complete the application on

Game Director: Nebraska State Soccer Association Staff and the Tournament Committee will be the

Volunteering: If you or anyone from your team would like to help and volunteer during the tournament please
contact the Nebraska State Soccer office at (800) 909-4458 or (402) 596-1616. Time slots can be schedule for
between teams’ games. There is never enough help!
