ECD Stakeholders Outcome Reporting Survey
The purpose of this survey is to gather information about the outcomes of community
capacity building work from stakeholders at local ECD tables – province wide. The survey
looks at three aspects of community capacity building work: Community Planning and
Coordination; Community Awareness and Mobilization; and Service Delivery.
Your response is confidential Your response will simply be grouped with other
responses to provide an overall look at the outcomes of the ECD work in your community.
The results of surveys in your area will be collated and sent back to your community
coordinator. It is expected that your ECD table will then discuss the findings and decide
what, if any, changes your table wants to make as a result of this discussion. A brief report
on how your table intends to use the findings, along with the details from your coordinator's
Outputs report will constitute an annual report to your ECD funders.
How to Complete the Survey:
The survey consists of a number of statements related to building community capacity for
ECD. You can choose to answer "Agree"; "Disagree", or "Don't Know". Please choose the
answer that most closely matches your opinion. If you don’t know the answers to some
questions, that’s OK; it is not expected that you will know the answer to every question. If
you have any questions about the survey, please contact the ECD coordinator who sent
you the link.
At the end of each section, click on "Next". When you reach the end, click on "Done". You
will know that your survey has been uploaded to the Survey Monkey site when you click
"Done" and find yourself at the Success By 6 website!
You will be able to complete only one survey, however you can return to a survey to edit
your existing answers or finish incomplete sections. If you return to an incomplete survey,
you will be taken to the place where you left off. Please try to complete your survey by the
requested date.
