ENROLLMENT CONTRACT AND LIABILITY WAIVER
TEAMS/GAMES
•
There are no refunds or credits given for missed games
•
Players are allowed to play on more than one team with permission from the hockey league director. The two teams
must be within one division of each other. For example, a player may play on D-1 and D-2, but not D-1 and D-3.
•
There will be NO discount given for playing on more than one team.
•
Games can only be prorated with consent of the hockey league director. This will only be allowed in special cases, such
as anticipated leave of absence prior to registering for the season, or for a medical reason.
•
You must play 4 games in a 14 game season to be eligible for playoffs.
CANCELLATIONS
•
Pickwick Ice Reserves the right to cancel scheduled games due to special events.
•
Team Rep will be notified of any such cancellations
•
Cancelled games will be rescheduled
REFUNDS/INJURIES
•
In the event an individual player wishes to cancel a season and receive a refund, a $50.00 cancellation fee will be
deducted from that refund.
•
Any amount refunded to the customer will not include previously attended games.
•
ALL refunds will be given at the end of the season. For teams that pay a team fee, see Team Fee Agreement below.
•
Any player who is out longer than 3 weeks with injury will need a doctor’s approval in writing to return to the Burbank
Senior Hockey League.
TEAM FEE AGREEMENT
•
The Team Fee is $4,200.00. The Team Rep must render payment IN FULL on or before the fourth scheduled game of
the season. No delays or extensions will be allowed. Team Fee is based on a maximum of 15 players which lowers
Dues to $280.00 a player. Any additional players Added to a Team Fee will need to pay $280.00 per player as
well.
•
Payment is absolutely NON-REFUNDABLE. There are NO EXCEPTIONS to this clause. It is understood by the Team
Rep and ALL members on the team roster that there will be no reason or action which will merit a refund or credit,
partial, or in full, from Pickwick Ice Center. Under NO circumstances will a group or individual find cause to demand
any refund or credit. Suspensions, injuries, change in schedule, movement of residence, nor any other reason will be
considered for refund or credit.
DUES/U.S.A. HOCKEY
•
Dues are $350.00 for players, $250.00 for team reps and $150.00 for goalies
•
Half of dues ($175.00) are due by the 2nd game. Full payment is due by the 7th game. After the 7th game, a $50.00 late
fee will be added.
•
U.S.A. Hockey is done online at usahockey.com. You are required to bring in the confirmation page.
•
The league does not prorate dues for vacations, suspension, etc.
SUSPENSION/FORFEIT RULE
•
Any player/team rep. abusing (verbal or otherwise) any rink employees, referees, score keepers or hockey directors will
no longer be allowed to play at Pickwick Ice. NO REFUNDS will be given if this rule is enforced.
•
Any player who receives 50 penalty minutes in a season will be suspended for 1 game. If the same player’s penalty
minutes rise to 75minutes, the player will then be suspended for the remainder of the season. Any player who has
received the 75 penalty minutes will need permission to return the next season. Said player will also be placed on
probation for that season.
•
Forfeit rule: Any team that fails to call the hockey director at least 48 hours before a scheduled game, or if your team
fails to show to a scheduled game without a makeup being scheduled game, the team will lose that scheduled game
without getting a makeup game.
•
Match penalties carry over to the next season.
•
Off ice fighting: Any players involved in a fight off the ice surface anywhere inside the arena, and/or on Pickwick
property will be suspended from the league for the remainder of the season and from the property itself for a minimum
of 60 days.
LIABILITY
•
I acknowledge that I may be held financially responsible for any damages I may cause, accidental or deliberate to
Pickwick Ice Center.
•
I further acknowledge that an inherent risk of personal injury exists caused by pucks, skaters, sticks, or other players
while participating in the sport of ice hockey and willfully assume the risk. I agree to hold harmless and faultless all
officers and representatives of the Burbank Senior Hockey League and Pickwick Ice Center, should injury occur. I
agree to all league rules and understand the late payment of league dues may result in my expulsion from the league. I
am aware that any unregistered player allowed to play on my team may result in the loss of (3) games and a fine from
the season’s schedule. BY SIGNING THE REVERSE SIDE YOU AGREE TO ALL THE ABOVE TERMS
