Housing Options
 Group Homes
 Adult Foster Care Homes
 Semi-independent Living
and Supported Living
 Financial Assistance
Group Homes
 24 hour a day, 7 days a week care.
 License is held by provider, not landlord.
 Typically, Community Mental Health (CMH)
contracts with the provider.
 Supplemental Security Income (SSI) or Social
Security Disability Insurance (SSDI) is given to
provider based on ability to pay rules and to
address the cost of care.
Group Homes
 Small Group Home
 No more than 12 unrelated
individuals.
 Large Group Home
 At least 13, but no more
than 20 unrelated
individuals.
 Congregate Facility
 More than 20 unrelated
individuals.
Adult Foster Care Homes
 Individual or care provider is licensed to
provide care in their “own” home. Special
note, may NOT be licensed!
 The license determines the number of people
that can live in the home.
 Individual may or may not have a contract
with CMH.
 SSI or SSDI is given to license holder to pay for
cost of care.
Adult Foster Care Homes
 “Family Home”
 Private Residence



No more than 6 unrelated
adults.
Provide care at least 5 or
more days per week.
Cost is outlined in
personal care agreement,
may exceed an individual’s
ability to pay.

Semi-Independent Living
 Supported Independent Placement (SIP)
 Community Mental Health county managed
program, individualized via the PCP.
 Supported Living Arrangement (SLA)
 Individually managed program dictated by the PCP.

 Individual budget is

developed based on Person
Centered Plan (PCP).
Individual hires staff, or
contracts with provider
SSI or SSDI goes toward
individual budget to help pay

 Provides a voucher connected to the unit.
 30%-40% of adjusted gross income=rent amount
