Conditions: Given the following: handheld pulse oximeter, reusable finger probe, and an antiseptic wipe. Standards: Time: 5 minutes. Student completed procedure in minutes. Accuracy: Satisfactory score on the Performance Evaluation Checklist. Performance Evaluation Checklist Trial 1 Trial 2 Point Value Performance Standards • • • Sanitized hands and assembled equipment. 䉯 Stated the purpose of disinfecting the probe windows. • • • • • • If necessary, connected the probe to the cable. 䉯 Explained why the arm should be supported. • • Selected an appropriate finger to apply the probe. • • Checked to make sure the patient’s fingertip is clean. 䉯 Explained what to do if the patient’s finger is cold • Made sure that ambient light will not interfere with the measurement. 䉯 Explained why ambient light should be avoided. Ensured the probe opened and closed smoothly and that the windows were clean. Disinfected the probe windows and platforms and allowed them to dry. Connected the cable to the monitor. Did not lift or carry the monitor by the cable. Greeted the patient and introduced yourself. Identified the patient and explained the procedure. Seated the patient in a chair with the lower arm supported and the palm facing down. Observed the patient’s finger to make sure it is free of dark fingernail polish or an artificial nail. Checked to make sure the patient’s finger is not cold. 173 Copyright © 2012, 2008, 2004, 2000, 1995, 1990 by Saunders, an imprint of Elsevier Inc. All rights reserved. Chapter 4 Vital Signs Trial 1 Trial 2 Point Value Performance Standards • • Positioned the probe securely on the fingertip. • Instructed the patient to remain still and to breathe normally. 䉯 Stated why the patient must remain still. • • Turned on the pulse oximeter. 䉯 Explained what to do if the oximeter fails the POST. • Allowed several seconds for the oximeter to detect the pulse and calculate the oxygen saturation. • Ensured that the pulse strength indicator fluctuates with each pulsation and that the pulse signal is strong. 䉯 Stated what should be done if the oximeter is unable to locate a pulse. • • Left the probe in place until the oximeter displayed a reading. The reading was identical to the evaluator’s reading. 䉯 Stated the normal oxygen saturation level of a healthy adult (95% to 99%). • • • • • • Removed the probe from the patient’s finger and turned off the oximeter. Completed the procedure within 5 minutes. Allowed the cable to lay across the back of the hand and parallel to the arm of the patient. Waited while the oximeter went through its power-on self-test (POST). Noted the oxygen saturation value and pulse rate. Sanitized hands. Charted the results correctly. Disconnected the cable from the monitor. Disinfected the probe with an antiseptic wipe. Properly stored the monitor in a clean dry area.
