PATIENT SURVEY 2015
Thank you to all patient who
helped with our Patient Survey.
The results were very positive
with 88% of patient’s saying that
they would recommend our
practice.

the waiting room. Please feel free
to take a look.
GP REFERRALS
If your GP arranges a referral
you will be asked to phone the
surgery to obtain further details.
It is very important that you do
this as your appointment will not
be processed if you don’t.
GP LETTERS
Please remember that if you need
a letter from your GP there may
be a charge. Please ask at

ORDERING YOUR
PRESCRIPTIONS
When ordering medication please
make sure you only order the
items you need.
If you don’t need a particular
item, please do not request it. It
can be requested again when you
need it.
REPEAT MEDICATION
Please remember we need
2 WORKING DAYS to process
your prescription request.
FRIENDS AND
FAMILY TEST
Have you taken the NHS Friends
and Family Test yet ?
It’s an important opportunity for
you to provide feedback on the
service you receive from the
NHS.
You can do this on-line or in the
surgery

GROUP
Would you like to have your say
and be involved in our Patient
Surveys? You will help identify
areas for development and
improvement at the Practice.
As this is a “Virtual” Group you
will need an e-mail address for us
to send communications to you.

MISSED
APPOINTMENTS
Do you find it frustrating when
you can’t get an appointment?
During the month of February
and March there were 125
missed GP and Nurse
appointments. Please let us know
as soon as possible if you are not
able to keep your appointment.
TEST RESULTS
Please call between 11am and
3.30pm to enquire about your
Test Results. Please allow at
least 5 working days before
contacting us.
CARERS
Do you care for a family member?
Do you have a carer yourself?
If so please let us know. We have
information and details of
support that is available.
Also take a look at the
information in our Carers Corner
in the waiting room.
Your Carers Lead at the surgery
is Cath. Please ask if we can be of
any help.
We are also offering Health
Checks for patients registered
with us who are carers.
