Activity
Usability is defined by ISO as "The extent to which a product can be used by specified users to achieve specified goals with effectiveness,
efficiency, and satisfaction in a specified context of use." The EHR is used by many different health care professionals, from Physicians and
nurses to clinical managers and administrators. To effectively accommodate different agendas, modifications can be made in the EHR. This
activity will explore some of those modifications
Start the Activity
Sign in to the EHR with your electronic signature on www.neehrperfect.com and double click on the icon labeled “EHR.” Use your Access and
Verify codes to sign in and access the Patient Selection Screen.

Usability
The Patient Selection Screen contains many options for filtering patient charts in the EHR to make it easier to locate and open the correct chart.
The next few steps will guide you through using of these options.
Skill: Selecting a patient chart by name.
1. In the patient selection screen type in the name “Unique”. How many other patients have this last name?
2. Click once with your mouse on the patient’s name to select the patient. Write down the DOB and SSN for this patient.

Skill: Selecting a patient chart with the same name.
3. Now instead type in “Meaningful”. How many patients have this last name?
4. Click once with your mouse on any one of the names to select a patient. What happens when you click on the patient?
5. Click once more on this patient’s name and describe what changes occur.
6. Record the name, date of birth and SSN number of the patient you have selected here. Describe the implications of having multiple
charts with the same name.
Skill: Exploring patient chart filters by location.
7. On patient selection screen, locate the filter options in the upper left-hand corner. They are in a section labeled Patient List. Which
filtering options are available?
8. Select the option to filter patient charts by Ward. Click on the various ward names. Describe what happens when you click on a
ward. Which ward has the most patients?
Skill: Selecting a patient using the SSN number.
9. Click on filtering option All. In the patient search box, type the last four digits of the SSN number of the patient with the non-unique
name from question 6 above. What happened?
10. Many health professionals prefer looking up patient charts by SSN or medical record number rather than patient name. Explain why
this may be important.
Optimizing the Views in the EHR
The views in the EHR can be very important to the clinician. In this section you will examine the views and how to change the preferences.
Skill: Noticing data fields displayed on the Cover Sheet
11. Go to the patient selection screen and type in your last name. Select your ePortfolio chart to open it.
12. The default display is the Cover Sheet. Notice the Tabbed organization along the bottom. What are the data fields displayed on the
Cover Sheet? **Hint: Active Problems is one of the data fields displayed.

Skill: Changing the font size to accommodate user preferences or devices.
13. On the Cover sheet go to the Edit tab on the toolbar. Choose Preferences then choose fonts. Adjust the font to different sizes
14. Adjust the coversheet (using click and drag in cells) to accommodate the new sized font. Experiment until you arrive at a layout that
balances between font size and view-ability. For example, large fonts will run over the edges of the cell, preventing the user from
viewing portions of data. Click through all of the tabs (problems, meds, orders, etc.) to examine the downstream effect of the
change.
15. Change patients – pick another inpatient from the available patient list (use Brady,Leeanne)
16. Were the adjustments you made in your portfolio retained in the new patient chart? Does the optimal view for your portfolio
translate into the optimal view in all tabs?
17. In a few short sentences, explain the potential trade-offs for clinicians of font size versus the need for increased scrolling.
Skill: Changing default presentation
18. Clinicians may want to change the default display. Go to the Tool tab on the toolbar and select Options.

19. In the popup box, choose General tab and then choose Other Parameters.
20. Change the field for Initial tab when CPRS starts to anything except “Cover”.

21. Choose OK then OK again to get back to cover sheet view. To apply the edit, you must exit the EHR and log back in. Discuss the
rationale for why a clinician would select the opening you choose when opening the EHR.
Cursor actions used in the EHR
There are four main cursor, i.e. mouse, actions used in the EHR: 1) the single left click to open menus or select data, 2) the hover, click, hold and
drag to resize, 3) the double left click to open reports, and 4) the right-click to access actions. Go to the patient selection screen and select
“Unique, Ima”

22. To open a report about content on the Cover Sheet, hover your cursor over one of the active problems, allergies, or clinical
reminders. Double click on any one of these data elements. Describe what appeared when you double clicked.
23. Open the Orders Tab by clicking on the tab labeled Orders at the bottom of the chart screen. Double click on one of the orders listed
in the Orders Tab. What type of report are you looking at? (**Hint: Report type is listed at the top of the report screen.)
Skill: Resizing and moving windows in the EHR.
24. With a report window open, place your cursor on the blue bar along the top of the report window. Left-click on the blue bar and
hold your left button on your mouse. While holding your left mouse button move the report window to the far left side of the chart
screen. Close the report window by pressing the “X” in the upper right corner of the report or pressing “Close” along the bottom of
the report window. Double click on the order to re-open the report window. Does the report re-open on the far left-hand side of
the chart screen? If not, where does it reopen?
25. With a report window open, place your cursor along the lower right-hand corner of the window border. Left-click on the window
border and hold your left button on your mouse. While holding your left mouse button drag the report window border to the far
lower right side of the chart screen to make the window larger or drag the window border up to the left to make the widow smaller.
Close the report window by pressing the “X” in the upper right corner of the report or pressing “Close” along the bottom of the
report window. Double click on the order to re-open the report window. Does the report re-open the same size it was when you
closed it?

Skill: Resizing Note Template windows in the EHR
26. Click on the Notes Tab. Double click on New Note to open the Progress note template search screen and type “Health” and open
the template called NURS: Health History. The template will load in a separate, pop-up window (called the “Boilerplate text).
Expand the template window to full screen. Use the Maximize button located in the upper right-hand corner of the template
window. Once the template is maximized, press “Cancel” to exit out of the template window. When you press cancel, you will
receive a Cancel Dialogue Processing message asking if you are sure you want to cancel without saving. Press “Yes.” Open a new
template window by pressing the “Change” button on the upper-right side of your Notes Tab screen. Search for “Care” and open
the template called “Clinical Assessment & Care Plan.” Did the Clinical Assessment & Care Plan template window open maximized?
27. Close out of this window by selecting Cancel. You will see a confirmation window, select YES. Your progress note will still be in
progress even though the template was cancelled. Now, right-click on the blank note (white space) and choose Delete Progress
Note… then select YES at the confirmation window.

28. Go to the Orders Tab, place your cursor on an order and perform a right-click with your mouse. What actions appear? (**Note:
These Order actions will be different for providers and non-providers or students and faculty).
29. While on the Orders Tab, select the Action Menu found at the top of the screen what additional Order actions are listed here?
30. Navigate from the Orders Tab to the Notes Tab in the chart. To open the Note action menu, right-click in the body of a note
(anywhere on the right-hand side of the screen). What menu options are available for Notes?
31. Compare the Action Menu at the top of the Notes Tab screen with the action menu available via right-click. Are there any
differences in the actions? If so, what are they?

34. Select Document Titles. In the Document Titles search box on the left type in “Assessment” and select Clinical Assessment & Care
Plan to add to your preferred list, or “favorites”, of notes. Highlight the note (s) and click Add.

35. When you have completed adding the note(s) titles click Save Changes then OK. Then OK again to leave the Options box. Now
when you go to the Notes Tab and click on New Note the note(s) you selected will appear at the top of the other notes in the EHR
above the dividing line.

36. Open the top FILE Menu in the upper left hand corner of the screen. Select EXIT.
