An introduction
The
current
picture in
Camden
Camden is a very diverse and complex borough.
While outcomes for local people in many areas
have been improving over the last decade,
there are still significant inequalities that need
to be addressed.
The local health and care economy is facing decreasing financial resources
at the same time as increasing demand for services.
In Camden, the care that people receive outside of hospital comes from many
different places, such as GP surgeries, community health centres, specialist
units and social services.
With around 230,000 people living in Camden and more than 100 health
and care agencies, it can be hard making sure that everyone works together
in the most effective way possible and that residents receive the best care.
We know that care works best when it’s joined up, because it offers improved
quality of service to individuals and makes better use of our resources.
With increasing demand for health and care services, we need services to be
more focused on prevention and supporting people to care for themselves,
rather than only treating people when they’re unwell.

Improving
for the
future in
Camden
Over the last three years, health and care
organisations in Camden have worked together
to improve services so that local people
experience better care and improved health
outcomes. There has been some important
learning from this work, which we’re now keen
to build on.
The Camden Local Care Partnership brings together the health and social care
organisations in the borough, to introduce a new single model (whole system)
for the delivery of these services.
Camden Clinical Commissioning Group (CCG) and Camden Council have
established the Camden Local Care Partnership with other health and care
providers. Our aim is to deliver a more united approach to local care and
a better quality of service, ultimately helping to deliver the best health for all.
This will help address the three national challenges identified by NHS England
in its Five Year Forward View:

A new
model
of care
We’ll achieve this by changing the way care is
organised locally so that it’s easier for organisations
to work together to deliver better care. We’ll also
be reviewing how care is delivered to make sure
it meets the following five key principles:
Accessible: Services that are easy to understand and access
in the right place at the right time.
Person-centred: Compassionate care that offers choice,
control and helps people to take responsibility for their own health.
Coordinated: Where multiple services are involved, they are
coordinated around the individual to provide a seamless care
experience and, where possible, a continuous relationship with
professionals involved in their care.
Preventative: At every level of need, care is focused on early
intervention to prevent escalation of need.
Effective: Evidence-based care that results in improved
outcomes for the best value.

• More care will be provided within the community, including more
specialist services.
• Primary care and community services will work together as one service.

How
we will
achieve
this
We’re reviewing local services to identify key
changes we need to make. Some of these
changes will help services run more smoothly,
for example: changes to IT systems and
contract arrangements.
Other changes will be more visible to the public,
for example: more services available in local GP
surgeries, or better information available on local
services and how to look after yourself. We won’t
be redesigning every service, but some will need
to change to make the whole system work better.
We believe it’s important that we plan these changes together, so we’re
involving people from all parts of the system, including residents, hospital staff,
GPs, social workers and commissioners. The Camden Local Care Partnership
will be overseeing all the changes and monitoring the impact to make sure they
are resulting in better care for local people.
The changes won’t all happen at once; we expect that it will take until 2021
for us to fully deliver the strategy. However, these ideas aren’t all new and we’re
already working on many projects that will improve care, including:
Camden Integrated Digital Records, which brings together
patients’ health and care information in a single digital record.
Camden Diabetes Integrated Practice Unit (IPU), which brings
together all services to achieve better care and support for people
with diabetes.
Camden Integrated Musculoskeletal (MSK) service, which brings
together GPs and specialist clinicians to deliver better results
and experiences for people with joint, muscle or bone conditions.

Working
with the
people of
Camden
The main driver of this strategy is to improve
care for local people, so it’s important that
Camden residents are involved at every
stage of the strategy. Patient and public
representatives form part of our partnership
and our decision-making process.
We have set up a public and patient reference group, which will provide ongoing
input into the strategy, but we’ll also consult relevant groups on proposals
relating to specific services as they develop. We’re also using a representative
citizens’ panel of more than 1,000 Camden residents to gather their views
on local services and what needs to change.
