FACIAL PROTECTION
The Malvern Old-Timers Hockey League purchases insurance for players through the Canadian Adult Recreational Hockey Association.
CARHA’s Facial Protection Policy states: Players wearing full facial protection or half-shield (visor) with mouth guard will be allowed to submit
a medical and/or dental claim for facial injuries. Member players not wearing facial protection equipment will not be insured for facial or
dental injury.
Please sign on the appropriate line below whether you wear facial protection – either a full face mask or visor.
Note: Visors without a mouth guard do not qualify for facial or dental coverage through CAHRA.

RETURNING PLAYERS - EARLY REGISTRATION:
We hope you have enjoyed playing in the MOHL, and plan to return next season. As a returning player, your fee to play
next year will be $385.00 .
IMPORTANT: MARCH 1, 2016 DEADLINE
Please check to ensure that the information shown on the registration form is correct and make any applicable
changes. Submit your signed registration form along with a cheque to your team captain on or before Tuesday,
MARCH 1, 2016. Please make your cheque payable to MOHL. It can be post-dated to June 1, 2016.
LATE REGISTRATION:
If your registration and cheque are not received by March 1, 2016, the league will assume that you have chosen not to
return next season, and will regrettably remove your name from the active players' list immediately. If you
subsequently decide to return to the MOHL you will be considered a 'new' player and will be subject to the new
player's fee (see below) and will be placed on the MOHL Waiting List.
NEW PLAYERS:
We are always interested in adding new players to the league. If you know of anyone who would be a good fit for the
MOHL, please ask him/her submit an application form to the address listed on Page 1. Upon receipt of a completed
Registration Form, new players will be immediately added to the MOHL waiting list, and assigned to a team roster at
the first appropriate opportunity.
The fee for new players to the league next season is $410.00 ($385.00 Annual Fee plus a onetime $25.00 Administration Fee)

It is the intent of the league Executive to draft all players to provide balance throughout the league and mix players
amongst the teams to avoid continuing groups on the same team each year.
All information must be complete to be accepted and printed in a readable format.
All Registration Fees deposited after June 1-16 are non-refundable, pending a decision by the league Executive.
Registered players agree to abide by all conditions imposed by the MOHL Executive now and in the future.
All decisions made by the Malvern Old-Timers HL Executive are final.
By signing this Registration Form, the Registering Player signifies that he has read and agrees to abide by the League
Constitution, a copy of which is posted on the league website at www.MOHL.ca.
10. By signing this Registration Form the Registering Player hereby releases, waives and forever discharges the Malvern
Old-Timers Hockey League, its Executive, players, referees, conveners, successors and assigns of and from any claims,
demands, damages, costs, expenses, actions and causes of action, whatsoever, whether in law or equity, in respect of
death, injury, loss or damage to the Registering Player or property, however caused, and the Registering Player further
agrees to indemnify and save harmless all the aforesaid from and against any and all liability incurred by any of them as
a result of, or in any way connected with participation in the Malvern Old-Timers Hockey League.
