This questionnaire must be completed for any application to a technical standards
committee and for any change in personal data of an existing registration. The
application process is described in the procedure ILNAS/OLN/Pr001 – “Procedure concerning
the registration to technical standards committees”.
The Luxembourg standards body undertakes to respect the confidentiality of information
provided in the questionnaire and other documents.
In case of change of personal data of an existing registration, the same form is to be
used. However, it is not necessary to complete items that remained unchanged since
the last form.
It is therefore up to the candidate to update the information mentioned in this questionnaire at
the Luxembourg standards body. This information will be available to the executive committee
“Standardization” of ILNAS for each decision on the registration of a technical committee, as
well as to the Luxembourg standards body for the assessment and review of the folder.
All the documents related to the functioning of the Luxembourg standards body can be found
on the website: www.portail-qualite.lu
