Every Las Lomas Senior must return this completed form with a valid parent/guardian
signature in order to purchase ticket(s) for the Ball.
All guests must also complete a form. If the guest is a Las Lomas student Form 2 must be
completed. If the guest is NOT a Las Lomas student, Form 3 must be completed and turned into
the Attendance Office for approval. All Forms are on the Class of 2015 website or in the
Attendance office.
The completed forms will be collected at the time of ticket purchase. Students will not be
allowed to purchase tickets without submitting all of the appropriate form(s).
Tickets will be sold in the Theatre Lobby at lunch on April 23, 28, 30, May 5 and 7. Cash & Check
only; checks made payable to Las Lomas Class of 2015
Tickets are also available via LL webstore. Forms must be submitted along with webstore receipt.
Scholarships are available. Please see Mr. Campbell if interested.
