In accordance with the Standard Code of Rules of the Football Association and League
Regulations of the KicKStart Youth Football League - Sunday, it is a requirement that
consent is given for the above player to compete in the League during Season 2017/18.
Failure to provide the signatures required under these rules and regulations will render
this registration form invalid until such time as it is fully completed to the satisfaction of
the League Registrar. Also in accordance with Rule 8 (A) (ii), by signing this form as Club
Secretary it is understood that your signature is confirmation that you have personally
checked the relevant ID of this player, and that by doing so their date of birth qualifies
them to participate in the age group stated above.

New Player Registration Form - Guidelines
All new players must be registered on a KicKStart Youth Football League - Sunday ‘New Player
Registration Form - Season 2017/18’.
New player registration forms can be downloaded throughout the season from
www.kickstartfootball.co.uk but not until 1st June 2017 onwards
Please complete the form clearly and accurately in black ink making sure that you have provided all
the information required and that the form has been signed by the Player, one Parent/Carer and
the Club Secretary.
Please note that it is not a requirement for Under 7’s or 8’s to sign the form.
Before returning the form please check you have got the following as listed in 1 - 2 to send in
with it:1.
A fee of £3.00;
2. One passport sized photograph of head and shoulders (with no hats or sunglasses).
Please note that any new player registrations received that do not contain all the required signatures
as underlined in bold above, and/or £3.00 and/or a passport sized photo will be invalid until such
time as it is completed to the satisfaction of the League Secretary/Registrar.
A new rule introduced this Season - Rule 8 (A) (ii) states Such that player ID data should not be
held by Leagues or uploaded to the Whole Game System. Clubs are now required to confirm that
they have checked the relevant ID and hold the data should inspection be required by Leagues. If a
potential overage player is reported to the League, this could result in the player being deemed
ineligible and the Club/Team being fined and/or docked points.
The form and enclosures can be sent by post or delivered by hand to
Elaine Pritchard – League Secretary/Registrar, 3 Hathersage Way, Sutton in Ashfield,
Nottinghamshire. NG17 2BE up until 3rd September 2017 and afterwards from the start of the
season on 10th September 2017, but with the option also to leave the form with your Site
Administrator on Football Reception any playing Sunday, for forwarding to the League Registrar.
The deadline for receipt of a player registration form every week is by 5.00 pm on the Friday
preceding the playing Sunday. This is necessary to allow enough time for the form to be checked, the
Player ID card to be printed and the Team Sheet to be updated.
When Player ID cards have been produced, they will be handed out initially at the League’s Induction
Meeting(s) either on 22nd August 2017 for new teams or 24th August 2017 for existing teams, but after
this they will be available for collection from your Football Reception on the following playing Sunday
from when the form was received (see deadline above).

NB: New player registration forms will be accepted throughout the season from 1st June 2017 up until
16th March 2018, unless there are exceptional circumstances; which would need approval by the
League Management Committee. This is a Football Association Standard Code of Rule not a League
Rule.
