FUNDING SECITON Instructions on how to complete Appointment Action Request Form Section 1: Header Section a. Once the creator opens the IAP, his/her information will auto-populate. Please verify information is correct. b. Select what type of action from the drop down menu. Section 2: Employee Bio Section a. Once the creator enters the new/re-hire individual’s EMPLID, his/her information will auto-populate. Please verify information is correct. b. If new/re-hire is for faculty – please complete “Tenure Status and Rank” boxes from drop down menu Section 3: General Information Section a. Use available drop down menu to complete section. b. Please provide as much information on the new/re-hire individual in the 2 note sections for appropriate authorization Section 4: Job Data Section a. b. c. d. Please complete all relevant sections. Job Begin and End Dates represent the actual dates during which an individual is expected to perform his or her assigned duties. Once creator enters Position ID, the rest of the information should auto-populate. Please verify information is correct. The Standard Hours, FTE, Rate Code, and Rate section must be completed by creator. Please verify information is correct, i.e. Annual Rate for staff, Academic Rate for Faculty and Students and use corresponding rate (i.e. if you chose academic rate, enter 9 months annual rate). Use drop-down menu to complete this section Section 5: Funding Section a. Once creator enters Cost Center/Project ID, other information is auto-populated. Please verify information is correct, i.e. cost center/project ID title is consistent with cost center/project ID number. b. Once information auto-populated ensure that the Job (appointment) begin and end date in section 4 fall within the Project begin and end date in this section 5. c. The funding begin and end date represents the active funding year for a cost center/project, and the Job (appointment) begin and end date in section 4 also must be within that period.0. d. Distribution Percent Time: This represents the percentage of overall costs a particular funding source will cover during the period for which an individual is appointed. a. All funding sources provided must total 100% for the period during which an employee is appointed.
