UNIVERSITY HEALTH & SAFETY COUNCIL MINUTES

Absent: Michael Luvera, Forrest McNeally, Andres Moreno, Jeff Penick, and Steve Sarchet
The meeting was called to order at 10:02 a.m. by Morgan Bliss, Chairperson; quorum present
1. Action Items
Approval of the minutes: A motion to approve the minutes of November 16, 2016 was presented by Jim
Hudson and seconded by Shane Scott. The motion was approved.
2. Information Items
Council Membership: Replacement of Betty Hawkins (Civil Service – WFSE) with another WFSE member,
preferably from Dining Services.
Review of 2016 Loss Trends: see attachment for further details.
 By Occupation
o Highest number of reported injuries (51) = FMD, including student employees (4)
o Second highest number of reported injuries (23) = Dining Services, including student
employees (11)
 By Cause
o Highest number of reported injuries (29) = Slips, Trips, & Falls (STF)
o Second highest number of reported injuries (25) = Cuts/Punctures
o Third highest number of reported injuries (24) = Material Handling (twisting, turning, bending
stooping, reaching)
 Student Employees
o  58 percent of reported injuries come from Dining Services (11)
o 42 percent of reported injuries are Cuts/Punctures (8)
o 4 of the 6 Burn/Scalds are student employees from Dining Services

2016 Loss Trend numbers are typical of past years
Dining Services has the highest percentage of student employees than any other department on
campus
— Dining Services has a high turnover of student employees each quarter ( 30 percent)
— Dining Services does have a safety committee but sporadic meetings throughout the academic
year
— Dining Services does perform standard training for its student employees (knife safety, safe lifting,
etc.)
Member Questions:
 Should we consider doing safety training as part of the hiring process?
 Should we have the various departmental safety committees report their
issues/concerns/findings to the University H&S Council each year, even once per quarter?
Online Reporting Forms
 Accident Reporting (see attachment “HOW TO COMPLETE AN ONLINE ACCIDENT REPORT FORM” for
details)
o Can locate the form by either searching in A – Z or typing accident report in search box
o Form is used by both employee/student employee and students/visitors. Each group is
identified by marking circle in upper left-hand corner of online form
o For employee/student employee accident report, supervisor is sent completed form by
individual to fill out their portion (i.e., time loss, corrective actions, etc.)
 Report an Unsafe Condition or Hazard
o Form allows for anonymous reporting (as per L&I)
Member Questions:
 Should we have departments post the “HOW TO COMPLETE AN ONLINE ACCIDENT REPORT FORM” on
break room bulletin boards?
 Is it possible to put the accident reporting form(s) in the CWU student app?
 EH&S will have its own webpage; currently under the FMD banner
Draft Safety Plan: (see attached document)
 Request H&S Council review and provide comments by the next meeting in January
Update on EH&S Manager Hiring Process
 In the interviewing Stage
 Four (4) interviews scheduled for this week
Follow-up Items: Jim Hudson is planning to email out the draft written programs (Hazard Communication,
Chemical Hygiene Plan, Accident Prevention, Personal Protection Equipment, and Respiratory Protection)
FTGOTO
 Fire Marshal is currently doing the biennial fire inspection (academic & business buildings)
throughout December
 OSHA’s Top 10 Violations (2016) shared with members (see attachment)
