Introduction
1. Reporting requirements in Victoria
Victoria’s bus operators are required to report certain types of transport incidents to
Transport Safety Victoria (TSV) under the Bus Safety Act 2009 (Vic) (BSA). TSV uses the
data it collects from bus operators to monitor trends and safety risks across the Victorian
transport network.
Accredited and registered bus operators are required to report bus incidents to TSV in
accordance with the requirements of the BSA and Bus Safety Regulations 2010 (Vic) (BSR).
2. Changes to information
The information contained in this report is the intellectual property of TSV and is year-to-date
(YTD) March 2017. It is sourced from its Occurrence Database, which stores data reported to
TSV by bus operators, as at 13 April 2017. The information might be adjusted to reflect new
information that comes to light at any time without prior notice. Consequently, minor
variations may exist when compared with previous publications.
Variations in information from year to year are evident in this report. Improvements in safety
are generally expected to occur over time as enhancements are made to systems. Low
frequency - high consequence events however, such as those occasionally experienced in
the transport industry, and changes to transport demand, such as increasing patronage, can
also result in variations in the information.
3. Definitions
Items listed provide a description of the different types of bus incidents that may be reported
to TSV.
3.1. Serious injury
An accident or incident that results in a person requiring immediate treatment as an in-patient
in a hospital1.
3.2. Collision
A collision with any person, vehicle, infrastructure, obstruction or object includes:
 accident near a bus, including collision with a passenger that, immediately prior to the
accident, alighted from a bus, or a collision between an intending passenger and
another road vehicle
 struck by missile
 collision between two or more buses
 collision between a bus and car
 collision between a bus and person
 collision between a bus and truck
 collision between a bus and bicycle
 collision between a bus and any road-side object (excludes collision with road or rail
bridge)
 collision between a bus and tram

operators will attribute ‘serious injury’ where, as a consequence of a bus incident, a person is
transported to hospital by ambulance.

collision between a bus and motorbike
collision between a bus and road vehicle where the road vehicle type is not specified
other collision, including collision with:
o a road or rail bridge
o a trailer that has become detached from a vehicle
o farm machinery
o an animal.
3.3. Slip, trip or fall
Any occurrence involving a person slipping, tripping or falling on or from a bus including
when boarding a bus, on board a bus, alighting from a bus, or being caught in bus doors.
3.4. Fire
Any occurrence of a fire on, or affecting, a bus which endangers or has the potential to
endanger the safety of bus operations, people and/or premises, including explosion or
implosion.
3.5. Suspected or attempted suicide
This definition includes any suspected or attempted suicide involving a bus.

4. Bus - Occurrences with injuries
This section provides data about bus occurrences that involved injuries.
4.1. Fatalities by person type
This section provides information about fatalities that occurred from 2015 to 2017. The data
excludes suspected suicides and natural causes.
Chart 1 shows the number of fatalities by person type (employee, passenger and member of
the public) that occurred in each quarter from 2015 to 2017 and the three year moving
average. This data is also represented in Table 1.
The total number of fatalities in:
2015 was 1 in the first, 3 in the third and 3 in the fourth quarters
2016 was 1 in the second and 1 in the fourth quarters.
The three year moving average remains at the same level with 1 in the first quarter of 2015
and 1 in the first quarter of 2017.
Employee fatality recorded in 2015 was 1 in the third quarter.
Passenger fatalities recorded in 2015 were 2 in the third quarter.
Public fatalities recorded in:
2015 were 1 in the first and 3 in the fourth quarters
2016 were 1 in the second and 1 in the fourth quarters.

quarter from 2015 to 2017.
Fatalities resulting from collision in:
2015 were 1 in the first, 3 in the third and 3 in the fourth quarters
2016 were 1 in the second and 1 in the fourth quarters.

4.3. Serious injuries by person type
This section provides information about serious injuries that occurred from 2015 to 2017. The
data excludes attempted suicides and natural causes.
Chart 2 shows the number of serious injuries by person type (employee, passenger, member
of the public and unknown) that occurred in each quarter from 2015 to 2017 and the three
year moving average. This data is also represented in Table 3.
The total number of serious injuries in:
2015 was 17 in the first, 10 in the second, 21 in the third and 22 in the fourth quarters
2016 was 11 in the first, 18 in the second, 14 in the third and 19 in the fourth quarters
2017 was 14 in the first quarter.
The three year moving average increased from 13.8 in the first quarter of 2015 to 15.2 in the
first quarter of 2017.
Employee serious injuries recorded in:
2015 were 1 in the first, 4 in the third and 1 in the fourth quarters
2016 were 4 in the first, 1 in the second, 1 in the third and 5 in the fourth quarters
2017 were 2 in the first quarter.
Passenger serious injuries recorded in:
2015 were 8 in the first, 6 in the second, 14 in the third and 9 in the fourth quarters
2016 were 6 in the first, 13 in the second, 9 in the third and 13 in the fourth quarters
2017 were 6 in the first quarter.
Public serious injuries recorded in:
2015 were 8 in the first, 3 in the second, 3 in the third and 12 in the fourth quarters
2016 were 1 in the first, 4 in the second, 4 in the third and 1 in the fourth quarters
2017 were 6 in the first quarter.
Unknown serious injury recorded in 2015 was 1 in the second quarter.

Table 4 shows the number of serious injuries by occurrence type (bus security and safety,
collision and slip, trip or fall) that occurred in each quarter from 2015 to 2017.
Serious injuries resulting from bus security and safety in:
2015 were 1 in the first, 2 in the second, 2 in the third and 2 in the fourth quarters
2016 were 2 in the first and 2 in the third quarters
2017 were 3 in the first quarter.
Serious injuries resulting from collision in:
2015 were 12 in the first, 4 in the second, 14 in the third and 19 in the fourth quarters
2016 were 5 in the first, 6 in the second, 9 in the third and 12 in the fourth quarters
2017 were 5 in the first quarter.
Serious injuries resulting from slip, trip or fall in:
2015 were 4 in the first, 4 in the second, 5 in the third and 1 in the fourth quarters
2016 were 4 in the first, 12 in the second, 3 in the third and 7 in the fourth quarters
2017 were 6 in the first quarter.

5. Bus - Occurrences
5.1. Collision with bicycle, motorbike and person
This section provides information about collisions with bicycle, motorbike and person that
occurred from 2015 to 2017.
Chart 3 shows the number of collisions by category type (bicycle, motorbike and person) that
occurred in each quarter from 2015 to 2017 and the three year moving average. This data is
also represented in Table 5.
The total number of collisions with bicycle, motorbike and person in:
2015 was 2 in the first, 4 in the second, 5 in the third and 6 in the fourth quarters
2016 was 4 in the first, 6 in the second, 10 in the third and 3 in the fourth quarters
2017 was 10 in the first quarter.
The three year moving average increased from 4.6 in the first quarter of 2015 to 4.9 in the
first quarter of 2017.
Collisions with bicycle in:
2015 were 4 in the third and 1 in the fourth quarters
2016 were 2 in the second quarter
2017 were 3 in the first quarter.
Collisions with motorbike in:
2015 were 1 in the fourth quarter
2016 were 1 in the first, 3 in the third and 1 in the fourth quarters
2017 were 3 in the first quarter.
Collisions with person in:
2015 were 2 in the first, 4 in the second, 1 in the third and 4 in the fourth quarters
2016 were 3 in the first, 4 in the second, 7 in the third and 2 in the fourth quarters
2017 were 4 in the first quarter.

5.2. Collision with car
This section provides information about collisions with car that occurred from 2015 to 2017.
Chart 4 shows the number of collisions with car that occurred in each quarter from 2015 to
2017 and the three year moving average. This data is also represented in Table 6.
The total number of collisions with car in:
2015 was 25 in the first, 21 in the second, 22 in the third and 32 in the fourth quarters
2016 was 24 in the first, 23 in the second, 26 in the third and 22 in the fourth quarters
2017 was 26 in the first quarter.
The three year moving average increased from 21.5 in the first quarter of 2015 to 25.6 in the
first quarter of 2017.

5.3. Collision with heavy vehicle
This section provides information about collisions with heavy vehicle that occurred from 2015
to 2017.
Chart 5 shows the number of collisions with heavy vehicle (bus and truck) that occurred in
each quarter from 2015 to 2017 and the three year moving average. This data is also
represented in Table 7.
The total number of collisions with bus and truck in:
2015 was 10 in the first, 3 in the second, 4 in the third and 4 in the fourth quarters
2016 was 8 in the first, 4 in the second, 5 in the third and 11 in the fourth quarters
2017 was 5 in the first quarter.
The three year moving average increased from 4.2 in the first quarter of 2015 to 5.7 in the
first quarter of 2017.
Collisions with bus in:
2015 were 3 in the first, 1 in the second, 1 in the third and 1 in the fourth quarters
2016 were 3 in the first, 2 in the third and 4 in the fourth quarters
2017 were 2 in the first quarter.
Collisions with truck in:
2015 were 5 in the first, 2 in the second, 3 in the third and 3 in the fourth quarters
2016 were 5 in the first, 3 in the second, 3 in the third and 6 in the fourth quarters
2017 were 3 in the first quarter.

This section provides information about slips, trips or falls that occurred from 2015 to 2017.
Chart 6 shows the number of slips, trips or falls by category type (caught in bus door, heavy
braking and on/from bus) that occurred in each quarter from 2015 to 2017 and the three year
moving average. This data is also represented in Table 8.
The total number of slips, trips or falls in:
2015 was 13 in the first, 12 in the second, 16 in the third and 15 in the fourth quarters
2016 was 21 in the first, 29 in the second, 15 in the third and 26 in the fourth quarters
2017 was 23 in the first quarter.
The three year moving average increased from 12.8 in the first quarter of 2015 to 16.8 in the
first quarter of 2017.
Slips, trips or falls resulting from being caught in bus door in:
2015 were 1 in the second and 2 in the fourth quarters
2016 were 1 in the second, 1 in the third and 4 in the fourth quarters.
Slips, trips or falls caused by heavy braking in:
2015 were 6 in the first, 5 in the second, 4 in the third and 7 in the fourth quarters
2016 were 6 in the first, 12 in the second, 6 in the third and 5 in the fourth quarters
2017 were 13 in the first quarter.
Slips, trips or falls on/from bus in:
2015 were 7 in the first, 6 in the second, 12 in the third and 6 in the fourth quarters
2016 were 15 in the first, 16 in the second, 8 in the third and 17 in the fourth quarters

5.5. Fire
This section provides information about fires on buses that occurred from 2015 to 2017.
Chart 7 shows the number of fires on buses that occurred in each quarter from 2015 to 2017
and the three year moving average. This data is also represented in Table 9.
The total number of fires on buses in:
2015 was 3 in the first, 4 in the second, 5 in the third and 3 in the fourth quarters
2016 was 3 in the first, 2 in the second and 8 in the fourth quarters.
The three year moving average increased from 3.1 in the first quarter of 2015 to 3.2 in the
first quarter of 2017.
