We are requesting innovative ideas from existing providers of General Practice services that meet the below criteria. Principles of the fund; The fund will not be split into sub localities for 14/15. This will be reviewed again in the context of co-commissioning The fund will help deliver the Essex Primary Care Strategy to bring about the transformation of primary care services. The fund will help deliver the CCG’s Integrated Plan The fund will be directed at areas where the greatest impact for patients is evidenced and support direct patient care. The fund will prioritise bids that clearly demonstrate how transforming primary care will impact on urgent care The fund will give priority to areas which are seeing the greatest growth in population or the greatest shortage in GPs/1000 population across Essex. The fund will give priority to areas of high deprivation. The fund will be available for the revenue costs associated with capital developments that support the delivery of the bid (not for capital developments) Priority will be given to groups of practices to bid for, not individual practices. The fund will be available to support the development of primary care premises. Applicants to the fund will already be providing access to patients 8:00am-6:30pm and will not be closed to patients lunchtimes or half days (closure for staff training/time to learn events agreed with the CCG are allowed and will not disadvantage bidders) Applicants to the fund will be operating the electronic choose and book system (directly or through a referral management service). Priority will be given to applicants to the fund who are signed up to the LD DES and Unplanned Admission DES The fund will be available for schemes both recurrent and non-recurrent in nature. The fund will set annual priorities which may differ from year to year in line with the Area Team’s commissioning intentions. The fund will welcome bids which are innovative in nature. Priority will be given to GMS practices and those PMS practices who have agreed the use of their PMS premium with the Area Team. Bids that are received that include a mixture of GMS and PMS practices who have not agreed the use of their PMS premium will only be accepted if those PMS practices make up 20% or less of the total number of practices submitting the bid Bids awarded will have an agreed start date and will be allowed a maximum of three months slippage after this date. If this timeline is exceeded, the application will become null and void. Most localities could benefit from improved access to training and support. This could be particularly useful for new GPs and nurses, but could extend to all clinicians. It could take the form of increased protected “Time to Learn” sessions, but in smaller localities, or specific courses and training on significant clinical and other issues in primary care. Building a responsive and accessible primary care service Proposals that redesign the delivery of services within a defined locality to ensure patients have improved access to primary care services, seven days a week, so reducing the pressure of unplanned activity in the acute sector. Practices could unite to develop a Locality GP centre where same day appointments could be seen by rotating GPs from a number of surgeries, or by additional GPs and nurses. The purpose would be to reduce pressure on appointments, but at the same time give a clear message to patients that this was an extension of their normal GP service and not something additional and experimental. Clearly access to records would be crucial. Wessex is preparing “Urgent care” centres along these lines but these centres do not have to be labelled in this way. They would probably only see patients of the participating surgeries and could cover populations of 20-50,000. We are seeking innovative solutions that change the model of how patients access primary care services. This could involve the use of new technology to provide additional access points for patients; it could offer patients alternative ways of accessing primary care in the community. Practices and community providers could contribute to a nursing development scheme where local nurses could share training and skills. This might extend to shared clinics where patients could access the full range of nursing within the area. Interaction with nursing and residential homes might also have a place within such a scheme. Scheme could also consider the role of community pharmacy, wider primary care professionals (such as district nurses) within teams as well as improved links with local authorities and the voluntary sector. Building greater self-care management amongst their patient population Proposals from a group of practices which implement new schemes that help vulnerable patients manage their own condition across the practice areas they cover. Patients with the same conditions but from a number of practices could host training and support classes and possibly group consultations where all could interact with a primary care professional. This could involve GPs, nurse, dieticians , physiotherapists, pharmacists etc. Schemes might involve additional access points for vulnerable groups that go beyond the traditional GP surgery. A common request from patients with long term conditions is for more education and support in order to manage their own conditions. Building a more robust primary care workforce The fund will support schemes that attract and retain the primary care workforce in areas of significant shortage. New posts which are tailored for new entrants to general practice could rotate across several practices in an area. This should give the flexibility and variety which appears to be what is initially required by new GPs. Clearly this could extend to nurses and other professionals and might include coaching and mentoring.
