How to fill in the New Player Registration Form
1 Please fill in all information in the second box.
2 The form must have 1 Passport size and quality photo attached with the player’s name,
date of birth and Parent/Guardian signature on the back.
3 Please remember to send in a COPY of the player’s Birth Certificate, Passport or Medical
card attached to the form.
4 The form must then be given to the nominated League Official named on the top part of
the form by 9PM on the Tuesday that you wish the player to be playing the following
Saturday. (The £2 registration fee must be paid before the card can be produced).
5 The completed form will then be checked and if all information is correctly filled in the
player’s registration card will then be produced for collection.
6 Any incorrectly filled in form will be returned to the club secretary and a new form will
have to be filled in.
7 NO temporary player forms will be produced by the league
Example 1
Form sent to nominated league official by 9pm Tuesday filled in correctly then player’s card will be handed
back to club that Friday (taking only 3 days to be completed) eg Tuesday 4th form in card back to team
Friday 7th
Example 2
Form received on Wednesday and filled in correctly then the player’s card will not be handed back until the
following Friday (9 days after the Wednesday that the form was handed in). eg Wednesday 5th form in
card back to team Friday 14th

WELFARE ON A FRIDAY BETWEEN 7.30 & 9pm
