JUNIOR PLAYER REGISTRATION – 2013/14
Please return the completed form and relevant fee to your Team Manager no later than 31st July 2013.
The registration form is a requirement of the club and any child who is not registered will not be
permitted to play. Fielding unregistered players is not tolerated by the FA and may result in penalties
being imposed against your team. Registration fees are £60 per player (£50 for additional siblings)
(cheques payable to Elvington Harriers AFC)1. Match fees (incorporating training fees) are payable on
the day. Match fees are a minimum of £2 per child per match (the match fee can be increased by
individual Team Managers, with suitable notice, dependant on additional requirements throughout the
season, such as all-weather pitch hire etc.)

Medical Consent

The Club will make every effort to contact parents/guardians in the event of any illness or
accident, however I have completed the medical details above and consent that in the event of
such an incident, any necessary treatment can be administered to my child by suitably qualified
persons. I understand that whilst team and club officials will take every reasonable precaution
to ensure that accidents do not happen, they cannot necessarily be held responsible for any
loss of or damage to property or injury suffered by my child.
Photographic Publication Consent

I give permission for photographs/videos to be taken of my child whilst participating in football at
Elvington Harriers (this includes team photographs) and whilst taking part in tournaments and
Club events, and that such photographs/videos can be used for the promotion of Elvington
Harriers, publication in match reports and newsletters.
Travel Consent

I give my permission for my child to travel with the Club/Team to matches/events away from the
Club.
(For all away matches, arrangements will be made to collect and return children to specific pick
up points. The Club’s responsibility starts and stops at these points. It is your responsibility to
arrange safe travel to and from the pick up points. If you are ever delayed in collecting your
child, please make every effort to contact your Team Manager or the escorting helpers so we
can discuss arrangements for your child. There may also be occasions when your child is
involved in planned and structured non-football activities such as a trip to the cinema, bowling or
participating in a fund-raising event away from the Club.)
Codes of Conduct – If either of the Code of Conduct Declarations are not ticked, the
Registration will not be accepted.

I have read the Club’s Code of Conduct for Parents/Carers and Spectators (which can be
found at www.elvingtonharriers.co.uk/codeofconduct or on the last page of these registration forms)
and understand my obligation towards the team, the opposing team, match officials and to the
Club and I agree to abide by its terms at all times.
I have read the Club’s Code of Conduct for Young Players with the player named on the
registration form and he/she understands his/her obligation towards the game and the team,
and will treat with respect the laws of the game, the opposing team, the match officials and to
the officials and spectators of both teams, and I agree to abide by its terms at all times.
As an FA Charter Standard Club, Elvington Harriers has Codes of Conduct in place covering Players, Coaches,
Team Officials and Spectators. The Club expects these Codes to be adhered to and enforced at each and every
game. In ensuring that the Code is adhered to, a verbal warning would usually be given to any individual deemed to
be breaking the code. If further breaches occur, a written warning would be issued and if this ultimately failed to
address the problem, the individual concerned would be asked to leave the Club or abstain from attending games.
The Club reserves the right to move away from this process in circumstances where there are serious or dangerous
breaches to the Code.

Spectators and Parents/Carers Code of Conduct:
We all have a responsibility to promote high standards of behaviour in the game and that’s why Elvington Harriers is
supporting The FA’s Respect programme to ensure football can be enjoyed in a safe, positive environment.
Remember children’s football is a time for them to develop their technical, physical, tactical and social skills. Winning
isn’t everything. Play your part and observe The FA’s Respect Code of Conduct for spectators and parents/carers
at all times.
I will:

Remember that children play for FUN;

Applaud effort and good play from ALL players (not just my own team) as well as success;

Always respect the match officials’ decisions;

Remain outside the field of play and within the Designated Spectators’ Area (where provided);

Let the Coach/Team Manager do their job and not confuse the players by telling them what to do;

Encourage the players to respect the opposition, referee and match officials;

Avoid criticising a player for making a mistake – mistakes are part of learning;

Never engage in, or tolerate, offensive, insulting or abusive language or behaviour.
I understand that if I do not follow the Code, any/all of the following actions may be taken by my club, County FA,
league or The FA:
I may be:

Issued with a verbal warning from a club or league official;

Required to meet with the club, league of County FA Welfare Officer;

Required to meet with the club committee;

Obliged to undertake an FA education course;

Obliged to leave the match venue by the club;

Requested by the club not to attend future games;

Suspended or have my club membership removed (where applicable);

Required to leave the club along with any dependents.
In addition:

The FA/County FA could impose a fine and/or suspension on the club.
Young Players’ Code of Conduct:
We all have a responsibility to promote high standards of behaviour in the game. As a player, you have a big part to
play. That’s why Elvington Harriers is asking every young player to follow the FA’s Respect Code of Conduct:
When playing football, I will:

Always play to the best of my ability;

Play fairly – I won’t cheat, complain or waste time;

Respect my team-mates, the other team, the referee or my coach/manager;

Play by the rules, as directed by the referee;

Shake hands with the other team and referee at the end of the game;

Listen and respond to what my coach/team manager tells me;

Talk to someone I trust or the club Welfare Officer if I’m unhappy about anything at the club.
I understand that if I do not follow the Code, any/all of the following actions may be taken by my club, County FA,
league or The FA:
I may:

Be required to apologise to my team-mates, the other team, referee or team manager/coach;

Receive a formal warning from the coach or the club committee;

Be dropped or substituted;

Be suspended from training;

Be required to leave the club.
In addition:

My club, County FA or The FA may make my parent/guardian aware of any infringements of the Code of
Conduct;

The FA/County FA could impose a fine and suspension against my club.
