Cost to affiliate with MeHJA is $20. Please enclose a check made out to MeHJA along with this
form and proof of insurance.
By submitting this form, I understand that by affiliating this show with MeHJA I will provide, at
least 14 days prior to the show, proof of insurance in the amount of $1,000,000 General
Liability which includes spectators on or about the grounds and must name MeHJA as an

I further understand that a copy of the prizelist will be sent to the MeHJA Secretary no less than
30 days prior to the show for review and approval. Additionally, I agree to send the MeHJA
Secretary the results of the show on MeHJA forms within 14 days after the show. Failure to
submit results timely and on MeHJA forms will result in fines and ineligibility to affiliate future
