DECISION SUPPORT USC APPLICATION
The Decision Support USC application can be access from the following Link: Decision

HOME PAGE
The home page of the USC application is made of the following parts:





Main Menu - will take you back to this Main Menu page and is available from every
section of the application
Logout – logs you out of the DS – USC Access Request Application and does NOT save
partially completed requests
User Access (Add/Remove) - Request access for a new user or additional access for an
existing user who is an university employee
Data Warehouse Application Account – Request an application account. Application
accounts only have direct ODBC access to the data and are typically used by other
application to download bulk data.
View Access Request – Remove the request you have submitted.
NOTES:
The DS-USC Access Request Application will log you automatically after 30 minutes of
inactivity. A partial completed request will NOT be saved.
The Back Button has been disabled in this application.

USER ACCESS (ADD/REMOVE)
Request to Add access for a new user or additional access for an existing user who is a
university employee.
The system will only allow you to request access for an employee with an active job.
Select User Access (Add/Remove)
Select University Employee radio button
Enter the Employee’s UIN and select Search

If the user’s First Name, Last Name and Email address are correct Select Confirm User
The USC User Request Access page will list all Previously Completed Access Requests that you
have submitted for the user.

Select the DS Job Category that most accurately reflects the user’s job function. You can only
select one DS Job Category per request.
Select the Data Subject Area from the drop-down list. The Data Subject Area corresponds to the
type of data (i.e., Finance, HR/Pay or Student) that the user needs to access data in the Data
Warehouse. (Hold down the CTRL key to select multiple areas.)
You can get additional Information about the Roles and Subject Areas from the Data Warehouse
Access Plan.

In the Intended Use of Data (Describe) box, briefly describe the user’s access needs. Please
include any special instructions such as ‘Trish needs UIC campus wide access to STU PreRegistration and Registration data’.
Under the UI Information Security Compliance Form, select the appropriate radio button to
indicate if there is a UI Information Security Compliance Form signed by the User on file.
Under the FERPA Training, if access to student data is being requested, has the User received
FERPA (Family Educational Rights and Privacy Act) training? Use the appropriate radio button
to respond. If student data is to be accessed, the USC is required to confirm that the User has
received FERPA training. If student data is to be accessed, and the User has NOT received
FERPA training, access WILL NOT be granted until the User has completed FERPA training.
When you have completed the form select the “Submit” button

Request to Remove access for an existing user.
You should request to remove access when longer requires some of their access but still requires
an account.
Select User Access (Add/Remove)
Select University Employee radio button
Enter the Employee’s UIN and select Search

If the user’s First Name, Last Name and Email address are correct Select Confirm User
The USC User Request Access page will list all Previously Completed Access Requests that you
have submitted for the user.
Select the previously Completed Request(s) you would like to remove.
Select Remove Requested Access

Prove a brief description for why the access is being removed.
Select Request Removal.
You will be asked to confirm you would like to remove access.
Select OK if you want to request the access be removed or Cancel the request is not needed.

email of the request will be sent to you. Users are NOT notified when access is removed.
If you need to submit another request Select Submit Another Request.

Request to remove an account for University employee who has left your department or the
University
You should request to remove all access when an employee has left your department or the
University. If the employee needs access sin their new position, the new USC should submit a
request for access

Select University Employee radio button
Enter the Employee’s UIN and select Search

Select Remove Account (User has left the Department or University)

Prove a brief description for why the Account should be removed.
Select Submit.

Request for Non-University Employees
The process for requesting access or removing access for Non-University employees is similar to
the University Employee Request.
Select Non-University Employee radio button.
Enter the Non-University Employee’s First and Last Name.
Select Search.

If the user is not found, their information will be returned.
If a user is not found Select Add New Non-University Employee.
Complete the Non-University Employee Entry Form, providing a UIN for the person responsible
for the Non-University Employee.
Select Submit.
The remaining steps for Adding access, Removing access or Removing an account follow the
same process as a University Employee and can be found on pages 4-18.

DATA WAREHOUSE APPLICATION ACCOUNT
Requesting access for a Data Warehouse Application Account
Application accounts only have direct ODBC access to the data and are typically used by other

Enter the name of your Application (i.e., Decision Support Password Reset Application)
Select the DS Job Category that most accurately reflects the user’s job function. You can only
select one DS Job Category per request.
Select the Data Subject Area from the drop-down list. The Data Subject Area corresponds to the
type of data (i.e., Finance, HR/Pay or Student) that the user needs to access data in the Data
Warehouse. (Hold down the CTRL key to select multiple areas.)

Describe the likely data download pattern to be used by the Application.
Describe the intended use of the data by the Application

Enter the UIN for the individual who will be responsible for the Application Account.
Select Search. Their information will be populated in the fields below.
Select Continue Grant Access.

Provide the UIN of any secondary users – those users who will have access to the Application
Account and password.
Provide the name of the person who will be responsible for the Application account security plan
to ensure appropriate downstream use of the data retrieved.
Provide ant Comments, suggestions or questions.
Under the UI Information Security Compliance Form, select the appropriate radio button to
indicate if there is a UI Information Security Compliance Form signed by the User on file.
Under the FERPA Training, select the appropriate radio button to respond. If student data is to
be accessed, the USC is required to confirm that the User has received FERPA training.
Read and Confirm the Application Account Guidelines.
Select Submit.

Review Request Status for University Employees
The Decision Support USC application allows you to review the status of your previous request
by University Employees, Non-University Employees, or Requested you have submitted.
Select View Access request
Select University Employee.
Enter the Employee’s UIN and select Search

All requests will be displayed. However you can limit the search results by using the Request
Status listed in the drop down box.

displayed without indicating for which user the request was submitted)
