FORMAT: TWO MAN TEAMS PLAYING - 6-6-6 Tournament
(Six Holes Scramble, Six Holes Best Ball, Six Holes Modified Alternate Shot)
PRE-FLIGHTED BY COMBINED HANDICAPS - 36 HOLES
TEE TIMES: Off #1 and #10 tees from 8 to 10 am & 12:30 to 1:30 pm
ENTRY FEE: $125 Per Player or $85 Per Crestwood Member
Entry includes: practice round, tournament play, tee prize, golf cart, players cook-out (11:30 am to 2:30
pm each day), and Saturday evening South American/Mexican themed food, beer and wine tasting
(includes players & spouse or guest) . Prizes (Pro Shop gift certificates).
OPTIONAL ENTRY FEE: $50.00 Per Team
Includes: Saturday Skins games, Sunday Skins Game, Draft a Pro from U S Open Sunday Round.

OPTIONAL GAME:
Two Man Scramble (Both men hit each shot until
ball is holed.)
Two Man Best Ball (Both men play the hole on
their own ball and the team takes the best score of
the two players.)
Modified Alternate Shot (Both men tee off, then
each man hits the others ball for their second shot
From there they pick the best of the two and
alternate until the ball is holed out.)
Saturday skins game, Sunday skins game & Draft
a Pro from Sunday US Open round @ $50 per team.
Each player must
provide a GHIN

club’s authorized
signature for handicap
or have a handicap
established through
Crestwood historical
data.
TOURNAMENT SCHEDULE OF EVENTS
Friday, June 18
Practice round. Call for tee times one week in advance. If you can’t play on Friday, use your
practice round at another time by calling Todd Casey (620-231-6530) for availability. Practice
Rounds expire July 30, 2010. A $13.00 cart fee will be due with practice round.
Optional Horse Race @ 3:00 pm. Horse Race is played as alternate shot. You must pre-register
on the form below and pay in advance for this event. Entry fee is $40.00 per team.
Saturday, June 19
Tee times 8 to 10 am and 12:30 to 1:30 pm
Entry is $125 per player, Crestwood Members @ $85.00.
Players cook-out from 11:30 to 2:30 pm,

Sunday, June 20
Tourney continues with tee times off #1 & #10 from 8 to 10 am for B, C & D flights.
Championship & A Flight off #1 & #10 starting at 12:30 pm.
Players cook-out at 11:30 to 2:30 pm.
Prizes awarded in the form of Golf Shop Gift Certificates. Number of flights & prizes determined by entries.
