1. Knowing that I am applying to be a role model to my peers and represent Pure Passion for
Fashion, Wedgwood Christian Services, and Kentwood Community Church I am
committing to not engage in risky behavior, such as sex (including all sexual behavior)
before marriage, substance abuse, and/or other risky behaviors.

2. Pure Passion for Fashion and Wedgwood Christian Services has the right to record my
child’s likeness through photo, video, audio as well as their artwork/written creative work to
be used with marketing materials including, but not limited to posters, news, television, and
websites for the promotion of Pure Passion for Fashion. These materials will be used in
world-wide marketing for Pure Passion for Fashion and Wedgwood Christian Services. I
release all rights to recorded media gathered during any shoot as related to Pure Passion for
Fashion and artwork created during activities.

3. I give permission for Pure Passion for Fashion staff and committee members to transport my
child for P2 related shopping trips and other activities, if necessary.

4. I understand that I am responsible for arranging transportation to all P2 rehearsals and
performances. Transportations for these dates will not be provided by P2 staff. P2 staff will
only provide supervision for 15 minutes prior to and following rehearsals.

5. I have reviewed the attached calendar and verified that I can commit to all mandatory
rehearsal and training times, including excusal from any conflicting extra-curricular
activities, if necessary. I have made note of any conflicts that directly affect rehearsal times
on the sheet provided.

6. I understand that the following will result in disciplinary action up to immediate removal
from the P2 Cast/Crew, including forfeiture of all participation opportunities, including
performances, cast party, and any earned incentives; showing up late repeatedly/no show,
violations of the P2 Commitment to Awesomeness, and/or other behavior deemed
inappropriate by P2 staff both at events, at school, or in the community. Parts/positions that
are offered are not guaranteed. Incentives are not guaranteed until work has been
satisfactorily completed.

7. Cast/crew that have accepted a part are expected to attend all rehearsals, trainings, and
meetings. Absences will be excused only in the case of urgent conflicts (sudden illness,
family emergency, etc). In case of an emergency, contact director Latesa Thomas, by
phone as soon as possible. Cast/Crew are responsible for checking with the appropriate
leaders in regards to other extra-curricular activities (sports, theater, church, appointments)
and communicate availability PRIOR to accepting a CAST/CREW position.

8. P2 may, at times, allow cast/crew to use P2 property such as clothing, props, and technical
equipment. Loss or damage to P2 property, due to negligence or improper handling, will
result in a fine equal to the price of the item.

9. If area schools are closed due to snow day or other emergency, all rehearsals will be
cancelled for that day. In the event of an emergency/closing on the day of performance, a
director will phone you to cancel or confirm whether the show will continue, as planned.
I have read this Commitment agreement with my child and we both agree to the terms/conditions listed
above. I understand that by signing this form, my child and I are signing a written contractual agreement for
their participation and commitment as a cast/crew member to the Pure Passion for Fashion Program and the
attached rehearsal dates.
