GP Walk-in Centres
If you cannot get an appointment with your own GP and have a minor injury or medical problem that is not life
threatening but you would like to see someone the same day. There are two walk in centres near our Plumstead
surgery and one near our Branch site in Welling. You will be able to see a GP, Nurse or Health Professional. NO
APPOINTMENT IS REQUIRED.

Ante-Natal
Ante-natal clinics are held by Midwives at The Plumstead Health Centre. We follow local Hospital Protocols for
Maternity Care. Ante- Natal booking is carried out by the doctors; please ask at reception for details.
We refer patients to The Queen Elizabeth Hospital Woolwich SE18. Post-natal checks (6weeks after birth) are
undertaken by the Doctor.
Pre-conceptual advice to couples planning a family, particularly to patients with pre-existing medical conditions, is
offered by Doctor. Doctors also advise on diet alcohol & substance misuse, supplements and Genetic disorders etc.
Contraception
Advice on contraception is given by all clinicians. Coil fitting and prescribing is undertaken by doctors only. Emergency
contraception is offered to patients who contact the surgery within 72 hours.
Baby Health Clinics
The Doctors carryout 6-8 week Baby Checks & immunisations are carried out by the Practice Nurse.
Parents are advised to book appointments at the required stages.
Other Services Available
Family Planning- coil fitting, etc- Minor operations, Phlebotomy Service is available at Plumstead on Mondays from
9.30am, by appointment only. Medicals for employment or for insurance purposes. Non NHS work is chargeable such
as private certificates, medical reports for housing, social benefits, insurance claims etc. Taxi and HGV medical
examinations, passport and driving license applications are chargeable.PLEASE ASK FOR FULL LIST OF FEES.
Repeat Prescriptions
Repeat prescriptions CANNOT be issued over the telephone. All prescription requests must be in writing or by ticking
the last printed script issued. AT LEAST 2 working days notice must be given. This gives the staff the time needed to
prepare and check that each prescription is correct. Faxed requests are accepted, if you wish to have your script sent
to you, you will need to enclose a stamped S.A.E. Some chemists provide delivery service to housebound and elderly
patients. We can organise this for you but non or late deliveries are not our responsibility.
Making Surgery Appointments
Please phone the surgery to book an appointment with the doctor or nurse. Remember the receptionist is here to
help you and cannot be held responsible for surgeries running late. If you cannot attend an appointment please ring
the surgery ASAP to cancel so that we can offer the appointment to someone else. Unexplained missed appointments
are not tolerated and may lead to removal from the list.
Home Visits
This service is ONLY for genuine emergencies or housebound patients. If you need a non urgent home visit please
phone the surgery before 11am usually the doctor will discuss whether a home visit is necessary. If you call after
11am the doctor may not be able to visit you until the following day.
Practice Nurse
We have a qualified Nurse who holds surgeries to advice on most ailments. She also runs a daily morning Telephone
advice and regular Chronic Disease management sessions such as diabetes, heart disease and asthma.
We recommend regular health checks. Please make an appointment to see the nurse for the following:
Cervical smears, child and adult immunisations, holiday vaccines, Health promotion e.g smoking, weight reduction,
asthma, diabetes , heart disease/stroke prevention, blood pressure checks, family planning advice, sexual health
advice and Over 75 health checks. Our nurse is also a Level 2 trained smoking cessation adviser. Doctors prescribe
Nicotine Replacement Therapy.
Minor Operations
We Specialise in Minor operations and have a dedicated operations room- Excision Lumps & Bumps, Vasectomy and

Complaints/ Comments and Suggestions
We always try hard to provide the best service possible but there may be times when you feel this has not happened.
The Practice adheres to a practice-based complaints procedure. If you have a complaint about any of the services
that we provide or a problem with a member of staff or any other issue you wish to discuss please contact the
practice manager, either in person or in writing. Full details will be taken and a decision is made on how best to
investigate the complaint.
We would also like to encourage patients to be part of our Patient Participation Group and make your views known
to us. Please ask at reception for details.
Practice Charter Standards
These are the local standards set within this practice for the benefit of our patients. It is our job to give you treatment
and advice. Following discussion with you, you will receive the most appropriate care, given by suitably qualified
people. No care or treatment will be given without your informed consent. In the interest of your health it is
important for you to understand all the information given to you. Please ask us questions if you are unsure of
anything.
Clinical Commissioning Group
As of 1 April 2013, The Waverley Practice will be included in the new governing arrangements for the NHS
and will be a part of the Clinical Commissioning Group (CCG).
New Patient Registration
Please bring your previous NHS card along with 2 proofs of address ie utility bills/telephone land line bill (mobile
phone bill is not acceptable). If you do not have your NHS card you will need proof of photo ID ie passport or driving
license plus 2 proofs of address. You will then be provided with registration forms. On registration you will be
required to book for a health check with the Nurse/HCA. It is advisable that all patients wishing to register with this
practice check that they are living within our catchment area - see practice boundary map attached.
Patients aged 75 and over are encouraged to attend for a health check on a yearly basis. Other patients who have
never had a health check are encouraged to book an appointment with the practice nurse or health care assistant.
Community Services
The following services are provided:- District Nurses, Community COPD, Community Gynaecology, Minor Surgery,
Cardiology and Dermatology. To be referred to these, please see the doctor.
Emergencies - Out of Hours Service
If you need Urgent medical assistance, and you cannot wait until the surgery re-opens please call 1-1-1. Calls to NHS
1-1-1 service is free from both landlines and mobile phones.
In life-threatening emergencies such as severe bleeding, collapse, unconsciousness and severe chest pains,
PLEASE TELEPHONE 999.
Confidentiality
We ask you for personal information so that you can receive appropriate care and treatment.
The information is recorded on computer and we are registered under the Data Protection
Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice
team.
Please note that only the patient themselves have the right to request copies of their medical notes. In all other
cases the person requesting must have the patient’s written permission and proof of ID will be needed by the
surgery.
Change of Personal Details
Please advise reception if you change your name (original documentation is required), address including postcode or
telephone number.
Disabled Access
Main site at Plumstead is fully disabled compliant and branch site at Welling is partially compliant. If you have any

Freedom of Information Act Publication Scheme
The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme.
A Publication Scheme is a guide to the 'classes' of information the practice intends to routinely make available.
Phlebotomy Service
The Primary Care Trust runs a phlebotomy service based here between 9.30 – 10.00am on Monday. Only a limited
number of patients will be seen by appointment only. For further information please speak to the receptionists.
Specimen Collection by Hospital Courier
If you are asked to bring in a specimen, please make sure that it is marked clearly on the bottle with the request form
given to you by the clinicians and both are placed in the plastic bags provided. Without this your specimen will not be
accepted. Please make sure that this is handed in at reception by 9.00am.
Electronic Prescription Service (EPS)
Electronic Prescription Service is a new service that will make it easier for GPs to issue prescriptions and more
convenient for patients to collect their medicines. Using EPS means that prescriptions by GPs and other prescribers
will be transferred electronically to the pharmacist nominated by the patient. The prescriptions will also be sent
automatically to the Prescription Pricing Authority. Please ask your pharmacist or at the surgery for further details.

Zero Tolerance/Staff Protection
A zero tolerance policy towards violent, threatening and abusive behavior is now in place throughout the National
Health Service. The staff in this practice has the right to do their work in an environment free from violence;
threatening and abusive behavior and everything will be done to protect that right.
At no time will any violent, threatening or abusive behavior be tolerated in this practice.
If you do not respect the rights of our staff we may choose to inform the police and make arrangements for you to be
removed from our medical list.















The Waverley Practice
The practice is in modern and purpose built premises with access for the disabled and
wheelchair users. We also have a branch surgery.
The Clinicians see patients by appointment only, unless in an emergency. Please cancel the
appointment if you are unable to attend. One appointment is for one person only.
Information about patients is kept on computers and is dealt with in strictest of confidence.
Under normal circumstances you will be seen by a clinician within 24 hours.
We will refer you to hospital where necessary.
Please do not expect a prescription every time you visit the surgery. Good advice is
sometimes the best treatment.
Doctors and staff do their best to help you. Please treat them with respect and politeness.
If you have a genuine complaint, contact the practice manager. Also, your positive comments
in appreciation of good service will encourage us to improve further.
Please notify us of any change of telephone number or address. We may need to contact you
in case of an emergency.
Please request home visit by doctor only if absolutely necessary- think twice you may receive
better treatment in the surgery under appropriate conditions.
Children under 14 must be accompanied by an adult.
Violent and abusive patients will be removed from the list instantly without warning or
notice.
Please remember that a lot of basic health information can be given by your local pharmacist
Please request repeat prescriptions in good time. To avoid any mistakes - minimum notice of
at least 2 working days.
Please remember that not all illnesses can be treated by a doctor- they are only human.
