IMPORTANT ANNOUNCEMENT

WE ARE MOVING! Yes, we've enjoyed this location on Brown Trail in Colleyville for
almost 8 years now. But the recent flooding next door to our office and resultant
flooding into our office has created concerns of possible risks that are too great
for our patient population.
And so, reluctantly, we have decided to move our office in November of 2013 to a
new building.
Beginning
November 11, we will begin
seeing
patients
at
789
Lonesome Dove Trail in Hurst,
TX 76054. This is only 3 miles
away from our current location,
so the change should not be
much of an inconvenience for
the majority of our patients. It is
off of Precinct Line Road, about
a quarter mile north of Glade
Road. Go north of the light at
Rumfield Rd., and Lonesome
Dove Trail is just a short
distance past the bridge.
We are excited to welcome you to our new office, and we are eager to continue
caring for you and your family! We know that you will be pleased with the quality
of care you will continue to receive at our new building. If you have any questions
or concerns about these changes, please call our office at 817-577-0480. Jenny
or Cindy will be happy to answer your questions! Look for notices at our office
during your visit, as well as mail outs with important dates and other information
about this change at our office.

DIRECTIONS
1. APPROACHING FROM
THE NORTH ON PRECINCT LINE ROAD:
1. BEGINNING in the area where Precinct Line intersects with Davis Blvd (1938),
GO SOUTH on Precinct Line Road. You will pass a Target and a Wal-Mart in this
area. It is about 1.8 miles from here to Lonesome Dove Trail.
2. CONTINUE SOUTH on Precinct Line past North Tarrant Pkwy.
3. CONTINUE SOUTH on Precinct Line past L.D. Lockett road. You will see a
Shell Gas Station on your left, as well as Bear Valley Community Church.
4. GET IN THE LEFT TURN LANE as you see White Chapel Estates Wedding
Chapel on your right.
5. TURN LEFT ON LONESOME DOVE TRAIL. Our office is the second building
on the right.
6. TAKE THE FIRST RIGHT into the drive that will lead you to our parking area. If
you miss the first right, that’s okay, the 2nd right will take you to the same area.
You will just have to drive past several other buildings to get back to us.

2. APPROACHING FROM
THE SOUTH ON PRECINCT LINE ROAD:
1. BEGINNING in the area where Precinct Line intersects with Hwy 183 (Airport
Freeway), GO NORTH on Precinct Line Road. It is about 3.0 miles from here to
Lonesome Dove Trail.
2. CONTINUE NORTH on Precinct Line past Harwood Rd. You will be passing
Wal-Mart shortly on your right.
3. CONTINUE NORTH on Precinct Line past Hwy 26 (Grapevine Hwy). You will
be passing Lowe’s shortly on your right.
4. CONTINUE NORTH on Precinct Line past Mid-Cities Blvd. You will be passing
The Home Depot shortly on your left.
5. CONTINUE NORTH on Precinct Line past Glade Rd, and GET IN THE RIGHT
LANE. You will be crossing railroad tracks immediately after the Glade
intersection.
6. CONTINUE NORTH on Precinct Line past Rumfield Rd.
7. TURN RIGHT ON LONESOME DOVE TRAIL. Our office is the second building

3. TURN RIGHT onto Mid Cities Blvd. You will continue on Mid Cities Blvd past
