Issue 459 26 January 2016 Issue 462 16 February 2016 Shining stars celebrated for achievement and long service Our awards season got off to a fantastic start on Friday 5 February with the Long Service and Staff Achievement Awards at the Castle Green Hotel in Kendal. Those staff with at least 25, 35 or 40 years’ continuous service in the NHS were presented with a badge and a certificate from Foluke Ajayi, Chief Operating Officer, to mark their loyal dedication to the NHS. Between them all, they racked up a massive 1,939 years’ service! They were joined by staff who were there to celebrate their achievements and qualifications, who were also presented with a certificate and a badge by Anne Garden, Non-Executive Director. The invitations for the Your Health Heroes awards have been sent and we look forward to seeing the finalists on Friday 18 March at the Castle Green Hotel, Kendal. Our Respiratory team, along with South Lakes Community Respiratory team, were presented with an award on Friday 29 January by Katie Merrick from the British Lung Foundation. This was as a result of winning the national ‘Hospital fund raising challenge’ competition under the name of ‘The Bay Breathers’. By undertaking various events with family and friends, the team raised just over £2,500. The events included the Cross Bay walk, a pub quiz, and a sponsored weight loss and beard shave! This was a great example of cross bay working. One in five people in the UK are affected by a lung condition. The British Lung Foundation provides support to patients, family and carers, as well as to healthcare professionals. It funds research for new treatments and campaigns for better care. Members of the respiratory teams have also been asked to form a Lancashire and Cumbria BLF forum, one of the first in the country, to help raise the profile of this respiratory charity. Ward 23 charity night to take place Ward 23 will be holding a Charity night on Saturday 27 February from 7.30pm until late. Please join them for a fun filled evening of entertainment at the Longlands Hotel, Carnforth. It includes charity auction, live music from Tears of the Dead, and a variety of burgers will be served. Tickets are £15 each and can be bought from Ward 23 or the Charity Office at the RLI. You can also now get raffle tickets for the event from Ward 23. Issue 462 16 February 2016 What does Governance mean to you? All staff should be familiar with Governance in their role such as incident reporting, investigations, audits, risk registers etc. We have devised a simple reporting framework to ensure we gather and report all Divisional information in a consistent and clear way, we call it WESEE (Workforce, Efficiency, Safety, Experience, Effectiveness). WESEE reports should be shared with staff at ward, departmental and Divisional meetings together with Divisional and Organisational Learning to Improve Bulletins. The post holder will be expected to work predominately Monday to Friday in an 80-20% supervisory capacity. They will have 24 hour responsibility with support from a new management structure providing seven day support from senior nurses. Anyone newly appointed to the trust will be subject to the completion of a satisfactory six month probationary period (this includes bank staff). This will give the new staff member an opportunity to become familiar with the role and working environment. Issue 462 16 February 2016 Keeping up to date with AKI Both Acute Kidney Injury (AKI) practice educators are continuing to train ward staff using new fluid balance charts at FGH and RLI. Thank you to those areas we have covered already for your time and patience during training. There is now an alert on Lorenzo for AKI within blood results under Renal Profile, please look out for this when reviewing blood results. If a patient has an AKI alert please ensure you use the kidney symbol available on the whiteboard to ensure everyone knows they need that little extra care. There is also a new AKI care bundle to use – anyone who has been alerted with AKI will need one. This is a team approach to AKI management where doctors, nurses and clinical support workers amongst others can all contribute! Any patient with an AKI during admission will need specific information included in their discharge summary. A prompt will appear on Lorenzo to remind you to complete the ‘AKI Discharge Plan’ (see above). Sepsis kills 44,000 people every year in the UK As part of our campaign to improve care for sepsis patients, we are participating in the national sepsis CQUIN for 2015/ 2016 (Commissioning for Quality and Innovation Payment Framework). This audits our compliance against the following parameters: Screening of all emergency admissions for sepsis using the sepsis screening tool, Administration of antibiotics within the hour for severe sepsis Whilst we have increased our screening rates from 22% in quarter 1 (April, May and June) to 51% in December, our average score for quarter 3 (October, November and December) was 46.9%, just missing the target of 50%. Our compliance with antibiotic administration has increased from 55.6% in July to 72.7% in December, our average score for quarter 3 was 66.7%, just missing the target of 70%. The target for quarter 4 (January, February and March) is 90% for both screening and antibiotic administration. This is a national Quality of Care target which as Trust we are failing to meet. What can we do? At the recent LiA Big Conversation about Health and Wellbeing, a question was raised about the need for assessments for staff using Display Screen Equipment (DSE) workstations and one of our Health and Safety Reps committed to taking the question forward. It was clear that not everyone knows about the policy and that DSE assessments are mandatory for anyone habitually using a PC, laptop or other DSE. So we put the DSE policy “on a page” to explain in quick and easy terms what’s required. This page will now sit at the beginning of the DSE Policy on the Procedural Documents library or by clicking here and also as an associated document on the library so it can be printed off and kept handy in the workplace. We hope you find it useful along with the other Policies on a Page the we have developed so far: Driving at work, Control of Contractors, RIDDOR, Manual Handling Training, Patient Handling and Ligature Management. Meeting the GMC standards and Health Education England requirements: To be fully recognised as a ‘Named’ Clinical or Educational Supervisor, trainers are required to document evidence from the domains, to demonstrate their engagement in educational activity. Gaining provisional and full recognition: Individuals who are listed on the HENW Trainer database as ‘Trained’ CS or ES will be provisionally recognised. To become fully recognised by July 2016 and to retain recognition thereafter each ‘trainer’ will need to have an annual appraisal which includes evidence of Educational CPD. This would be demonstrated by providing one piece of evidence covering domain 7 (CPD) per annum, with 3 separate types of evidence in any 5 year period Anyone not on the HENW trainer database and new Consultants/Trainers will need to attend a taught course that is externally accredited e.g. by Academy of Medical Educators, a Royal College or HEI to prepare them for the supervisor role. In addition new trainers will need to provide a minimum of 1 piece of evidence from each of domains 1 – 4 for CS and 1 – 6 for ES to become fully recognised. To retain recognition trainers are required to undergo annual appraisal which includes evidence of educational CPD. This would be demonstrated by one piece of evidence covering domain 7 (CPD) per annum, with 3 separate types of evidence in any 5 year period. Help with the quality and speed of your mail processing We often receive complaints that our outgoing mail is delayed resulting in missed appointments. Invariably any delays are caused by letters that are not folded correctly so the address fits in the window of the envelope, or where Post Codes are missed off altogether. There are some very basic steps which can minimise delays, save us money and most importantly ensure patients receive their appointment notifications in good time: All letters and/or envelopes MUST be printed Hand Written Mail will NOT be processed and will be returned to the Ward/ Department All letters MUST have a fully visible Post Code under the last line of the postal address There should only be a legible address showing. If the letter is in a window envelope any words/phrases such as ‘Dear John’ or ‘Private and Confidential’ must not be visible. Do NOT use sticky tape to seal envelopes. This fouls up the franking machinery. Our postal contractor has provided us with basic information on letter processing, which can be read on the communications page of the intranet or by clicking here. Issue 462 16 February 2016 Health and Safety: making it simple but doing it well! Last year, the Health and Safety Team held a series of ‘mini’ Big Conversations with a large number of nursing colleagues at their Study Days. The energy and engagement was incredible and the outcomes will result in improvements for all colleagues. As a result ,Health and Safety have produced an improvement plan for 2016. Health and Safety Documentation’ - you said: ‘ Simple, essential paperwork that makes a difference No repetition of risk assessments No duplication of care plans and notes What we are doing: Over the next 12 months, Health and Safety will be redesigning how the Trust manages risk assessments. Our intention is to continue to use a centralised risk assessment database similar to the COSHH Inventory, for all types of risk assessments E-care plans are currently being implemented and this too will remove duplication and repetition and maintain quality ‘Support from others’ - you said: More visibility of H&S team on ward Security support always available immediately What we are doing: The Health and Safety Team is providing managers support visits for all ward and Department Managers to give detailed and tailored support to each Manager. Respiratory masks provision - important information Recently, there has been some confusion regarding respiratory mask provision and we have now agreed with stores a new system whereby we will guarantee access to masks and maintain a single central stock which we will then re-stock through a cross charge provision to ward/clinical area budgets. From this, please ask the person responsible for ordering masks to contact the stores team on their respective site and call off masks from the central stock, they will need to provide the ward/clinical area budget code. On the back of this we now have over 120 trained mask fit testers across the Trust. Stuart Hosking-Durn is now in possession of ten new testing kits and a large stock of test solutions, so he can now equip local teams to test their own staff.
