Pharmacy Local Professional Network NHS England [Merseyside Area Team] Annual Work Plan 2014/15 with indication of priority level and expected lifespan The LPN aims to inform and support the implementation of national strategy and policy at a local level, work with key stakeholders on the development and delivery of local priorities and provide local clinical leadership. Our LPN will deliver its aims through clinical leadership and working in alignment with LPCs, CCGs, LAs, HWBs, neighbouring LPNs, the LPN assembly (see below) Priorities will be based on local need and national priorities and approved by the LPN steering group and will feed directly into the Area Team primary care commissioning and strategy. Priority 1: Clinical Engagement [Include in 3 year plan] The LPN through its first annual conference will be launching communities of Practice (CoPs) to develop clinical engagement amongst all clinicians involved with the care of patients. The engagement will include patients and the ultimate aims of the CoPs will be to deliver a patient focus and friendly service design/pathway. Priority 2: Effective Commissioning of services through pharmacy: [include in 3 year Plan] The LPN will act as independent adviser and scrutiniser of all current and future pharmacy commissioned services where public money is used to fund the service. The LPN will not be involved in the contractual negotiations, but will be happy to assist the commissioners and providers to reach agreeable conclusion. The LPN through the CoPs will forward innovative services to be considered by the commissioners. Priority 3: Healthy Living Pharmacies (HLPs) [Include in 3 year plan] The LPN will provide support and encourage Public Health, Local Pharmaceutical Committees, NHS England Local Area Team and others to adopt HLPs model and implement their role out. Priority 4: Pandemic Flu [I year plan] LPN will support NHS England and LAT to deliver its Pandemic Flu plan. Priority 5: Research in Pharmacy Practice [Include in 3 year plan] LPN will support and encourage pharmacy teams to get involved in research projects and will share amongst the network any funding streams that can be accessed by them. What is Pharmacy Local Professional Network Background Recent changes to NHS structures created a single NHS England with 27 local area teams to deliver the NHS strategy locally through existing local health economy. Pharmacy, Dental and Ophthalmic professionals were recognised by the NHS England as integral part of NHS in primary and secondary care. The Pharmacy LPN once set up is empowered to: • • • • • Support the implementation of national strategy and policy at local level Work with key stakeholders on the development and delivery of local priorities Provide local clinical leadership Advise and support NHS England, Public Health England, Local Authorities in evaluation, assessment and commissioning of services Act as the only local, impartial clinical engagement vehicle accessible to all the commissioners involved in commissioning any services with pharmacists and or pharmacies The work of LPN should ultimately deliver: • • • • • • • • Quality improvement Setting up local standards Best outcomes for patients that reflects local need Best use of NHS resources, supporting the delivery of QIPP Planning and designing integrated care pathways Clinically robust and appropriate commissioning Leadership and engagement Feed into National assembly What does all this mean in practice? Pharmacy teams deal directly or indirectly with the local population regardless of the setting, and are not necessarily the only people dealing with them. Hence the need to identify and work with all the stakeholders in NHS, Private enterprises, Local Authorities and Public Health organisations to ensure the best care for the population. Dealing with the population in a smooth pathway will ultimately result in better care and a better journey for them. The impact of this panacea of seamless working will ultimately lead to positive outcomes, through improved patient safety, reduction in medication errors, medicine optimisation, and better patient care amongst others. On our footprint [Merseyside Area Team] the care and services to the population is provided through NHS England and a number of CCGs, Local Authorities, NHS trusts and Community pharmacies. We are fortunate to have many proactive and leading edge pharmacists delivering high quality services. We need to identify and engage with these individuals, whose talent needs to be nurtured and shared. Through positive engagement with stakeholders to develop new or better pathways that should ultimately lead to better commissioning of existing and / or newly commissioned services. The PLPN steering group is establishing working relationships with all key stakeholders and encourages any interested parties or organisations to link in with the work of the PLPN.
