Tarikh:……………………… LAMPIRAN A SKOP PERKHIDMATAN PERKHIDMATAN PEMBERSIHAN DI INSTITUT KANSER NEGARA (IKN) PUTRAJAYA DEFINITION OF CLEANSING Cleansing is a process which removes foreign material (e.g. soils, organic material, microorganisms) from an object (e.g. floor, wall, ceiling, furniture) to render it safe. OBJECTIVE 1. To standardise cleaning procedures and schedules for Institut Kanser Negara (IKN), Kementerian Kesihatan Malaysia. 2. To create an impressive and hygienically clean image of IKN, KKM. SCOPE OF WORK 1. To provide cleansing management which include. 1.1 General Areas All Corridors All Lobbies Hospital Street Staircases Lifts Toilets (respective wards and public) Offices (Including administrative, medical record, medical social work, specialist office, IT, Health Education, Rehabilitation Office, QA/Research etc) Stores Kitchen (Hospital) Pantries Balcony Pavements, foot-paths Parkings Area (External and Internal) All other areas used by public 1.2 Medical Areas Clinics - Examination Room. Treatment Room Waiting Room Wards Pantry - Toilets/Bath-Room All Wards. of the respective wards - of the respective wards Independent Living Units, Patient Assessment, Common Therapy, Common Shared Facilities. Any other rooms around the respective ward environment. All other clinical areas used by public and staffs. 1.3 Specialised Areas Radiotherapy & Oncology Nuclear Medicine Laboratories Pharmacy Central Sterilization Supply Unit Imaging and Diagnostic Isolation rooms Operation Theatre TCM Dietetic and certain Kitchen Area Any other rooms in the specialised areas. 1.4 Other Areas All Specialist clinics (within the contract hospital boundaries). - Laundries Seminar Rooms Auditorium Medical Stores Admission Revenue Central Registration All stores Telefonis room Guard Room / Post / Area Library Porterage Physiotherapy, Gymnasium, Paliative Home Cancer Care Unit Quality and Research Unit Health Promotion Unit - Medical Social Service Volunteer Programme Unit On-call complex Mortuary 2. Shall include collection and transfer of general (non-clinical) waste from the above areas to the central storage facilities for such wastes in IKN. The Contractor shall also ensure that the general waste is being disposed off from contract hospital by the Local Authorities. 3. Hospital staff to segregate waste and place them in suitable and recognisable colour coded bags and containers. 4. To supply according to contract adequate number, various sizes of black plastic bags and containers for on-site bag holder of general wastes and also to supply suitable and adequate number of storage containers at the central storage facilities for general waste. 5. To provide management, equipment and chemicals for the cleansing of the above said areas and waste collection management in accordance with the Operating Procedure. All procedures proposed or used in order to implement the cleansing services shall be approved by MOH and complies to all statutory and legal requirements. 6. All equipment, chemical and disinfectants used for cleansing shall follows the manufacturer’s instruction and be approved by MOH. 7. To provide provisions and washing facilities according to contract at sinks i.e. detergent and detergent dispensers, paper hand/roll towels and its dispensers and bag holders lined with plastic bags. CSP 001 A. General Areas 1. Corridors, Balcony and Staircase 1.1 Objective : To ensure a clean and hygienic environment is being maintained in IKN, which shall be dry and free from bad odours, surface stains, litters, dust and other foreign matters in accordance to MOH requirements. 1.2 Scope : To follow and adhere to the scope as mentioned in the MOH requirement. 1.3 Definition : Corridor / Walkway Common space provided for patients, staff and visitors to get access from one place to another. Stairways - Common staircases provided in the general areas for patients, staff and visitors to get access from one floor to another. 1.4 Reference: MOH Technical Requirement for Cleansing Services. 1.5 Responsibilities: Contractor’s supervisor :- a) To guide and supervise the cleansing performance of the Contractor’s cleaners at their respective work place. b) To ensure that the cleansing services are carried out in accordance to the technical requirement of MOH. Lifts 2.1 Objective: - Refer Schedule and Frequency of Cleansing Services (Appendix 2.1 of MOH TRPI) To ensure a clean and hygienic environment in a lift is being maintained in IKN which shall be dry and free from bad odours, surface stains, litters, dust and other foreign matters in accordance to MOH requirements. 2.2 Scope: To follow and adhere to the scope as mentioned in the MOH requirement. 2.3 Definition: Lifts Mechanical equipment used for raising and lowering users and other tools and equipment from floor to floor activated by electricity. 2.4 Reference: MOH Technical Requirement for Cleansing Services. 2.5 Responsibilities: Contractor’s supervisor a) To guide and supervise the cleansing performance of the Contractor’s cleaners at their respective work place. b) To ensure that the cleansing services are carried out in accordance to the technical requirement of MOH. Stores 3.1 Objective: - Refer Schedule and Frequency of Cleansing Services ( Appendix 2.1 of MOH TRPI ) To ensure a clean and hygienic environment of a store is being maintained in IKN which shall be dry and free from bad odours, surface stains, litters, dust and other foreign matters in accordance to MOH requirements. 3.2 Scope: To follow and adhere to the scope as mentioned in the MOH requirement. 3.3 Definition: Store - A place made available for all storage purposes. 3.4 Reference: MOH Technical Requirement for Cleansing Services. 3.5 Responsibilities : Contractor’s supervisor a) To guide and supervise the cleansing performance of the Contractor’s cleaners at their respective work place. b) To ensure that the cleansing services are carried out in accordance to the technical requirement of MOH. Pantries 5.1 Objective: To ensure that a clean , hygienic and a satisfactory environment of a pantry is being maintained in IKN which shall be free from bad odors, surface stains, litters , dust and other foreign matters in accordance to MOH requirements. 5.2 Scope: To follow and adhere to the scope as mentioned in the MOH requirement. 5.3 Definition: Dining 5.4 - A place provided for dining in a canteen of any hospital or hostel. Pavement/Footpath / Parking Areas (External and Internal) 6.1 Objective: To ensure a clean and hygienic environment of pavement / footpath is being maintained in IKN which shall be dry and free from bad odours, surface stains, litters, dust and other foreign matters in accordance to MOH requirements. 6.2 Scope: To follow and adhere to the scope as mentioned in the MOH requirement. 6.3 Definition: Pavement / footpath - Common space provided for patients, staff and visitors to get access from one place to another. Toilets 7.1 Objective: To ensure that a clean, hygienic and a satisfactory environment of a toilet is being maintained in IKN which shall be dry at all times and free from bad odours, surface stains, litters, dust and other foreign matters in accordance to MOH requirements. 7.2 Scope: To follow and adhere to the scope as mentioned in the MOH requirement. 7.3 Definition: Toilet - A place made available for end users with facilities for washing and for bowel movements. 7.4 Reference : MOH Technical Requirement for Cleansing Services. 7.5 Responsibilities : Contractor’s supervisor a) To guide and supervise the cleansing performance of the Contractor’s cleaners at their respective work place. b) To ensure that the cleansing services are carried out in accordance to the technical requirement of MOH. Drains Within Hospital Building 8.1 Objective: To ensure that a clean, hygienic and a satisfactory environment of drains,floor traps and manholes is being maintained in IKN which shall be dry and free from bad odours, litters, dust and other foreign matters in accordance to MOH requirements. 8.2 Scope: To follow and adhere to the scope as mentioned in the MOH requirement. 8.3 Definition: Nil. 8.4 Reference : MOH Technical Requirement for Cleansing Services 8.5 Responsibilities : Contractor’s supervisor a) To guide and supervise the cleansing performance of the Contractor’s cleaners at their respective work place. b) To ensure that the cleansing services are carried out in accordance to the technical requirement of MOH. Central General Waste Collecting Area 9.1 Objective: Services ( To ensure that a clean , hygienic and a satisfactory environment of a central general waste store is being maintained in IKN which shall be dry and free from bad odours, surface stains, litters , dust and other foreign matters in accordance to MOH requirements. 9.2 Scope: To follow and adhere to the scope as mentioned in the MOH requirement. 9.3 Definition: Central Waste Store A place to hold all general waste collected from the contract hospital, for subsequent disposal by Local Authorities. It shall also be equipped with washing facilities . 9.4 Reference : MOH Technical Requirement for Cleansing Services. 9.5 Responsibilities : Contractor’s supervisor a) To guide and supervise the management of the store. b) To ensure that the store is in accordance to the technical requirement of MOH. (b) Waste generated All wastes which is general (non-clinical) wastes. 4.0 References : MOH Technical Requirement for Cleansing Services. 5.0 Responsibilities : a) Contractor’s Supervisor To guide and supervise the collection and transfer of general waste from point generation to the central store facility in accordance with the technical requirement of MOH. b) Contractor’s Cleaner To carry out the collection and transfer of general waste from point of generation to the central store facility. c) MOH Staff To segregate the waste accordingly . CSP 006 1.0 SEGREGATION OF WASTE Objective: The procedure is prepared to facilitate the clinical and non-clinical staff of MOH Institutions to carry out segregation of waste into clinical and non-clinical waste produced from their respective departments/wards, etc. 2.0 Scope: This procedure applies to clinical and non-clinical waste generated from all MOH Institutions 3.0 Definitions/Notes: Segregation of hospital waste into clinical and non-clinical is essential to ensure that clinical waste is treated and handled properly to avoid any possible hazard and cross infection to hospital staff, patients and the public. Clinical waste which is segregated will be stored in yellow bag holders which will then be collected by the Contractor for disposal.While the non-clinical wastes shall be collected using black bags and dispose off during the scheduled collection of wastes. 4.0 5.0 References: Management of Clinical and Related Waste in Hospital and Health Care Establishments, MOH, July 1992. Disinfection and Sterilization Policy and Practice, MOH Third Edition, 1994. Responsibilities: Clinical and non-clinical staff who produce clinical and non-clinical waste are responsible for segregation of the waste. However, overall responsibility to ensure segregation is carried out lies with the Head of the Institutions 6.0 Procedures: Waste segregation would involve the following procedures i. Identify whether the waste is clinical or non-clinical (Staff should refer to CWMS 0000 for definition of clinical waste) ii. After determining that the wastes are general (non-clinical ), the waste should immediately be deposited into the black bags in the bag holders provided for collection and disposed by the Contractor during scheduled collection. CSP 007 ON SITE BAG HOLDERS 1.0 Objective: To describe the procedures for handling the on-site bag holders . 2.0 Scope: To follow and adhere to the scope as mentioned in the MOH Technical Requirement for Cleansing Services. 3.0 Definitions: On site bag holder: The bag holder for general waste suitably positioned at appropriate locations within a ward, department and general areas. 4.0 References: i. 5.0 MOH Technical Requirement for Cleansing Services. Responsibilities: The Contractor’s cleansing porters shall be responsible to upkeep and maintain the onsite storage at their respective location. The contract hospital’s director or management will give the approval to the Contractor on the placement of the bag holders throughout the hospital area. MOH’s Officer will monitor the Contractor’s operation. 6.0 Procedures: i. The bag holders shall be neatly lined with clean black bags before being put into use. ii. Any bag holder found to be stained shall be removed and cleaned at the central general waste’s store or an identified area approved by the contract hospital director . There shall always be a bag holder at each of the approved designated location whenever one is being removed. iii. All bag holders shall be free from any pests. CSP 008 1.0 MANAGEMENT OF SPILLAGE Objective: The procedure is prepared for clinical and non-clinical waste within the contract hospital. 2.0 Scope: This procedure applies in accordance with MOH requirements and also to management of clinical and non-clinical waste spillage. 3.0 Definitions/Notes: Any splash (spillage) of clinical or non-clinical waste must be effectively handled to prevent the possibility of cross-infection to other patients , members of staff or the general public. 4.0 References: MOH Technical Requirement for Cleansing Services. 5.0 Responsibilities: Contractor’s cleaners shall attend to clean up spillage within the contract hospital upon receiving any requirement regarding spillage. The response time to act upon such instruction by user, is 10 minutes (maximum) from the instruction given. 6.0 Procedures: 1. Use personal protection clothing and emergency equipment as gloves, apron. 2. Warn any persons which may be at risk. 3. Close off area with plastic tape and warning sign. 4. Contain spillage - Disinfection using chlorine based chemicals. available e.g. 5. Clean up spillage and place collected wastes into yellow / black bags discard in bins. and 6. Mop with soap and water / clean water. CSP 009 COLLECTION ROUTE, FREQUENCY AND TIMING OF COLLECTION 1.0 Objective: To ensure that collection route taken, frequency collection of general wastes follows the schedule (timing) approved by MOH. 2.0 Scope: To follow and adhere to the scope as mentioned in the Technical Requirement for Cleansing Services. 3.0 4.0 Definitions: Collection route and timing : These are the ‘passageway’ in the contract hospital setup and the scheduling that is approved upon by MOH management at local site. Collection frequency : This is set out in Appendix 2.1 of TRPI document. References: i. 5.0 MOH Technical Requirement for Cleansing Services. Responsibilities: The Contractor shall discuss and propose to the respective contract hospital management on the route and timing for collection of general wastes. The contract hospital’s director or management will give the approval to the Contractor before the program is implemented. MOH’s Officer will monitor the Contractor’s operation. 6.0 Procedures: i. General waste in black bags shall be collected daily or as frequently as set out in App. 2.1 of TRPI. ii. Collection of general waste shall be carried out following an agreed time and via designated routes and using suitable devices dedicated for such collection. On no account shall general waste be collected with other wastes, or materials especially clinical wastes. iii. The routing and timing of collection shall be decided after consultation with the Hospital Director. iv. When formulating collection routes, the following important factors need to be considered: i. Minimise passage through patient care areas and other clean areas - subject to approval of Pengarah Hospital. ii. Try to avoid collection during visiting hours - subject to approval of Pengarah Hospital. iii. Avoid passage of waste through food preparation areas - subject to approval of Pengarah Hospital. iv. Avoid busy areas and public areas, if possible - subject to approval of Pengarah Hospital. v. Details of collection route shall be documented and submitted upon taking over of the general waste collection services by the Contractor. CSP 010 1.0 IDENTIFICATION OF RESPONSIBLE STAFF Objective: To enable easy identification of the cleansing staff. 2.0 Scope: The procedure shall apply to all MOH contract hospitals/ institutions. 3.0 Definitions: Nil 4.0 References: Nil 5.0 Responsibilities: The Contractor’s cleansing services supervisor shall ensure all cleansing staff observes the identification procedures approved by MOH. 6.0 Procedures: i. All janitorial staff shall be in uniform and carry special pass / card for easy identification. ii. All Contractors’ staff shall be registered with the contract hospital management. CSP 011 1.0 SEALING AND HANDLING OF BLACK BAGS Objective: To ensure that all black bags are properly tied before removing the bags from bag holders. 2.0 the Scope: To seal and handle all black bags from the bag holders at generation points or area serviced . 3.0 Definitions: Black bags 1985. - black plastic bags to contain general waste meeting BS : 6642 : On-site bag holders - a bin or receptacle which is used for collecting general waste. 4.0 References: i. 5.0 MOH Technical Requirement for Cleansing Services. Responsibilities: Contractor’s staff shall remove, seal with a tie all used general waste bag.The collector has to ensure black bags are properly tied before removal. 6.0 Procedures: i. Contractor’s collector will ensure that any given bag holder is three quarter filled. ii. After ensuring that the container is three quarter filled , the collector will then tie the bags properly so that no spills will happen during removal. iii. The properly tied black bags will then be removed from the bag holders and will be placed into a waste collection device for disposal. CSP 012 1.0 TRANSFER OF SEALED BLACK BAGS FROM ON-SITE BAG HOLDERS INTO COLLECTION DEVICES. Objective: Transfering sealed black bags from on-site bag holder into the collection devices. 2.0 Scope: This procedure applies to all general waste generated from all of the areas serviced in the Ministry of Health’s hospitals. 3.0 Definitions/Notes: Black b -black plastic bags to contain general waste meeting BS: 6642:1985. On-site bag holder -a bin or receptacle which is used for collecting general waste. Collection device 4.0 5.0 -a wheeled bin /trolley for collecting and transferring bagged general waste from the area serviced to the central general waste storage . References: i. MOH Technical Requirement for Cleansing Services. ii. B.S. 6642. 1985 Responsibilities: Contractor’s staff shall remove, seal and transfer the sealed black bags from the general waste bag holders into the collection devices. 6.0 Procedures: i. When handling or transferring black bags, hold them by the closure or neck end only. ii. Do not support the bag at its base with the free hand. iii. Bags are to be handled with care and not thrown or compacted into the collection devices. CSP 013 1.0 LINING THE GENERAL WASTE BAG HOLDER WITH BLACK BAGS Objective: To line all general waste bag holder with black plastic bags. 2.0 Scope: This procedure applies to all general waste generated from all of the areas serviced in the Ministry of Health’s hospitals. 3.0 4.0 Definitions: Bag holder - a rigid container, bin or receptacle for holding general waste. Black bag - black plastic bags to contain general waste meeting BS : 6642 : 1985. References: i. MOH Technical Requirement for Cleansing Services. ii. British Standard 6642:1985. 5.0 Responsibilities: Contractor’s staff shall line the bag holders with black bags.The black bag used shall be of adequate size for the bag holder. 6.0 Procedures: i. Ensure that the bag holder is clean before lining. ii. The Contractor’s staff shall line general waste bag holders and the upper rim shall be folded out such that the bags will only be filled up to 3/4 of its volume. CSP 014 MOVEMENT OF COLLECTION DEVICE WITHIN HOSPITAL 1.0 Objective: To determine the appropriate route for the collection device. 2.0 Scope: This procedure applies to all general waste collected using the collection device from all of the areas serviced in Ministry of Health’s hospitals. 3.0 Definitions: Collection device - is a mobile (wheeled) containers to be used for collecting filled black bags from the on-site bag holders to the central general waste store. 4.0 References: Nil 5.0 Responsibilities: The Contractor’s cleaning supervisor shall ensure all general waste collection porters take the approved routes. 6.0 Procedures: i. The Contractor shall discuss with the specific hospital management to determine the appropriate route to be taken by the general waste collection porters. ii. The Contractor shall then brief the relevant porters on the approved routes. iii. Should there be any obstacle at the approved routes, the porter shall use his/her discretion to continue his collection as long as it is not intrusive to the hospital services. He shall seek advice from the Contractor’s cleansing supervisor before taking any alternative route as his regular route. The Contractor’s cleansing supervisor shall inform hospital’s management immediately of the alternative routes taken. CSP 015 1.0 CENTRAL STORAGE FOR GENERAL WASTE Objective: To set-up a central storage area for general waste. 2.0 Scope: The procedure shall apply to all MOH contract hospital. 3.0 Definition: Central storage is the area designed and constructed exclusively for storing general waste prior to collection by the local authorities. 4.0 References : MOH Technical Requirement for Cleansing Services. 5.0 Responsibilities: The Contractor shall use the existing central storage, if suitable, according to local authority’s requirements. Otherwise the Contractor shall propose to MOH to modify the central storage or build a new one. MOH will consider the proposal and approve the central storage to be modified or constructed following the local authorities requirements. 6.0 Procedure: 1. The Contractor will review the suitability of the existing central storage for general waste. It shall be retained, modified or replaced if necessary upon the MOH approval. 2. The storage area shall be provided with the necessary utilities for its operation. 3. The Contractor shall upkeep its cleanliness and carries out its regular maintenance. The storage area shall be covered, well ventilated and free of pests. CSP 016 1.0 CLEANING OF ON-SITE BAG HOLDERS AND COLLECTION DEVICES AT CENTRAL STORE. Objective: To clean the on-site bag holders and collection devices. 2.0 Scope: This procedure applies to the bag holders and collection devices of general and clinical waste generated from all Ministry of Health Hospitals / Institutions. 3.0 Definition: On-site bag holder - Collection devic 4.0 black bag for a wheeled bin/trolley which is being used for collecting and transporting out all sealed black bags (general waste) from areas serviced to the central storage. References: i. 5.0 - a bin or receptacle which is lined with collecting general waste. MOH Technical Requirement . Responsibilities: Contractor’s staff shall clean all the bins / receptacles / trolleys according to frequencies as set out in Appendix 2.1 of MOH Technical Requirement for Cleansing Services. 6.0 Procedures: i. The Contractor’s staff shall clean all on-site bag holders and collection devices used following the frequency as set out in App. 2.1 of TRPI using water and approved detergent. Where necessary, disinfectants shall be used. ii. The Contractor’s staff shall wipe dry the on-site bag holders and collection devices prior to reuse. iii. The cleaning of the bag holders/collection devices shall be carried out at central store or suitable washing site within the contract hospital site , meeting the MOH’s requirements and approved by MOH . CSP 017 HEALTH AND SAFETY AND HOUSEKEEPING FOR CLEANSING SERVICES 1.0 Objective: To provide guidance on safe working conditions and environment for all employees, visitors and members of the general public so far as is reasonably practicable. 2.0 Scope: This procedure applies to cleansing services and it’s associated equipment and facilities. 3.0 Definitions: All reasonable, practicable steps should be taken to reduce risk of health and safety to employees, visitors to facilities and the general public. 4.0 References: Occupational Safety and Health Act, 1994. 5.0 Responsibilities: It shall be the duty of the Contractor to formulate and implement a health and safety policy. The Contractor will be required to provide appropriate information, instruction, training and supervision to create a safe working environment. 6.0 Procedures: The Contractor shall ensure that all reasonable, practicable steps are taken to reduce risk to employees and visitors to any facilities, equipment, areas or vehicles owned and/or operated by the Contractor. To attain this objective, the Contractor should be responsible for ensuring that appropriate information, instruction, training and supervision necessary to create a safe working environment, are provided for all employees. It is the duty of every employee to take reasonable care for the health and safety of themselves and their fellow employees, or any other persons who may be effected by their actions whilst at work or in the workplace and to co-operate with any other employee to achieve statutory requirements. Employees of the Contractor will have access to a copy of the health and safety statement, or policy, which may be reviewed and amended as conditions change. Further guidance is to be issued on the detailed specific arrangements for the organisation and monitoring of health and safety matters. Housekeeping A high standard of housekeeping is to be maintained at all sites during storing, collection and transporting the general waste out from the central store by Local Authorities. Materials and waste arising from cleaning of spillage from stored waste or during transport shall be placed in suitable bags/containers and subsequently disposed of. All cleaning equipment shall be maintained and operated according to manufacturer’s instructions. They shall be stored in a safe place accessible to Contractor’s employee. Refill detergent. APPENDIX 2 DEFINITION AND PROCESS. 1 . Damp Wipe Definition: To clean up surfaces without having to splash wet and leaving scratches around the area affected. Procedure: Use a Double- Basin Method or alternate system approved by MOH. (Clean water rinse from one and the cloth wrung out in the other after use). 1. Two basins filled 1 : 1 with detergent and water 1 : 2 with clean water 2. Water to be changed, for every room / whenever necessary. 3. Use different cloth for different room. Technique: Soak dry clean cloth into detergent and water Wipe the areas to remove dirt. Using another clean cloth , rinse and wipe surface dry. After wiping / cleaning : Wash basin with detergent, rinse and keep dry . Cloth for cleaning , to disinfect after use. Wash and dry the cloth. 2. Spot Cleaning Definition: To remove mark / stain / spot from surface. Procedure Damp wipe. Spray cleaning solution onto the surface to remove stain ,wipe, clean and dry. 3. Wash and Scrub Definition: To clean up dirty area by scrubbing with soap and water. Procedure Wash with soap and water. Scrub with scouring pad / brush . Rinse clean with water. Wipe dry the area . 4. High Dusting Definition: To perform the task of removing dust from furniture, walls and ceilings. Procedure Use vacuum cleaners / Treated dust control mop. 5. Vacuum Cleaners 1. Clean according to the manufacturers recommendations. 2. Use cleaner with air-filtering characteristics to prevent air-borne contaminants. 3. Change filter when necessary. 4.
