On Friday, February 13, 2015, Broward County nonprofit organizations will be recognized by the community they serve for their tireless efforts. At the 5th Annual South Florida Community Care Network Non-Profit Academy Awards hosted by Seminole Hard Rock Hotel and Casino and presented by 2-1-1 Broward, the collective efforts of Broward County non-profits and the accomplishments of non-profit leaders will be recognized at the Awards ceremony and luncheon. 2-1-1 Broward is now accepting nominations for outstanding individual and organizational efforts in nine categories: Arts, Board Leader, Collaboration, Collective Impact for Youth, Innovation, Lifetime Achievement, Non-Profit Leader, Organization of the Year, and Rising Star. Both young and established 501(c)(3) organizations are encouraged to submit an application. The community will select the winner of the Community Choice Award by voting online for their favorite organization. Up to three finalists will be selected in each category by a panel of impartial judges. Finalists will be revealed at the VIP Announcement Party. Winners of the awards will be revealed at the Awards ceremony and luncheon. Winning individuals/organizations will receive a $1,000 grant to their organization. Up to two finalists per category will each receive a $500 grant to their organization. Please note that grants awarded for the collaboration category will be divided equally among the collaborating organizations by the submitting agency. Finalists will receive publicity and awareness of the work of their organization through event press, media materials, newsletters and social media coverage. Qualified applicants not named as finalists will be entered for a chance to receive the Publix Community Choice Award. 2-1-1 is the first call for help for Broward County residents and the gateway to health and human services in our community. Last year, 2-1-1 Broward connected nearly 101,000 Broward County callers with over 4,000 different programs that were available to help them. Nominations must be received by 5:00 pm on December 1, 2014 – Email completed package to [email protected]. Finalists will be announced at a VIP Announcement Party to be held on January 15, 2015. Each nominee will receive at least one ticket for the organization to attend the invite only VIP Announcement Party. Winners will be announced at the Awards ceremony and luncheon on February 13, 2015 at Seminole Hard Rock Hotel & Casino (visit www.211-broward for non-profit ticket pricing). All nominees must meet the following criteria: Individuals/organizations must have a physical presence in Broward County and regularly deliver services to residents of Broward County. Individuals/organizations may self-nominate or be nominated by others. Organizations may be nominated in only one category. Past winners cannot be nominated again in their winning category but may be nominated in a different category. Past winners of the Publix Community Choice Award will not be eligible for public voting. The cover page and all questions for the appropriate award category must be completed in full. Include a copy of the organization’s 501(c)(3). Include a copy of the latest 990 form OR the latest audited financial statements and management letter (if applicable).
