INSTRUCTIONS for Completing the BU Terrier Card Request Form
This form is used to request a BU Terrier (ID) Card for non-regular staff (temporary employees,
temporary agency personnel, temporary faculty and research staff not eligible for orientation) assigned to
the Charles River Campus.
This form must be completed in full prior to coming to Human Resources.
To receive a Terrier Card, the individual must bring this completed and signed form along with a valid,
government issued form of identification to the Charles River Human Resources office, Monday through
Friday from 9:00 AM - 12:00 PM or 1:30 PM – 4:30 PM
This form should be completed and signed by the appropriate Vice President, Dean, Director or designee.
The individual will have his/her photo taken in Human Resources and will be issued a Terrier Card.
If the “End date of assignment” section is left blank the Terrier card will be inactivated six months from
the “Effective date of assignment” – unless the individual has an active BU payroll record.
It is not necessary to complete this form for full and part time staff who are hired through Human
Resources.
It is not necessary to complete this form for regular Faculty members who are scheduled to attend new
employee orientation (unless the faculty member needs his/her Terrier Card prior to the week he/she is
scheduled for Orientation).
International Employees/Scholars
All non-US Citizens, non-permanent residents, must visit International Students and Scholar Office
(ISSO) for the creation of BU ID number, and must receive written confirmation of their BU ID number
from (ISSO). A copy of the ISSO letter, or ISSO email, confirming the BU ID number must be attached
to the BU Terrier Card Request Form in order to receive a Terrier Card.
All international employees who will receive financial compensation from BU, must go to the ISSO
within three (3) days of commencing employment in order to complete Form I-9 Employment Eligibility
Verification. The ISSO is located at 888 Commonwealth Ave., 2nd floor, and I-9’s are completed during
the following times: Tuesdays 9:00-10:00 AM, Wednesdays 4:00-5:00 PM, and Thursdays 1:00-2:00
PM. In addition to providing valid employment authorization, international employees must also bring
proof of a BU academic appointment or job offer letter to ISSO.

A Terrier Card is not necessary to request an email account, SAP access, UIS access, or Kerberos
password. Please see your departmental Data Security Administrator, Payroll Coordinator, or
other departmental designee to request access.
