The PQQ should be completed by the lead consultant only. Please answer all of the following questions.
This PQQ should be submitted as both an Excel document (completed Excel template) and a PDF (inclusive of appendices if
applicable). Teams may change the layout of the PDF version if they wish, but please ensure that submitted answers are in the order
given, with clear indications of any appendices/supplementary information.
This PQQ will be assessed alongside the two quality questions (Q2 and Q3 of the Submission Requirements as set out on page 48 of
the Expression of Interest document (Team Composition and Relevant Experience).
The scoring matrix for all questions is set out on pages 50-52 of the Expression of Interest document. This PQQ will be assessed as set
out below.
Please note, in accordance with Part 5 Regulation 59 (1) of the Public Contracts Regulations 2015, suppliers may supply a European
Single Procurement Document (ESPD) in placement of evidence of meeting the criteria identified below.
