AK CLEVELAND ELITE FC WAIVER, RELEASE AND INDEMNITY AGREEMENT

In consideration for the acceptance by AK CLEVELAND ELITE FC of the above-named player, and his/her parent’s or guardian’s
request and or application to participate in events, athletic or otherwise, and sponsored, sanctioned, or participated in by AK
CLEVELAND ELITE FC and the provision of coaching of the Player by AK CLEVELAND ELITE FC, the player and his/her parent or
guardian (“parent”) undertake and agree as follows:
1. Player and Parent acknowledge that there are certain inherent dangers associated with participation in AK CLEVELAND ELITE
FC Events, including without limitation, participation in indoor and outdoor soccer practices and competitions and travel to and
from these practices and competitions.
2. Player and Parent acknowledge that AK CLEVELAND ELITE FC does not provide or maintain insurance of any kind whatsoever,
including without limitation, insurance which would cover the cost of medical, dental, or therapy arising from participation in a
AK CLEVELAND ELITE FC Event.
3. Player and Parent acknowledge that the Ohio Youth Soccer Association North does not provide insurance for among other
things, injuries arising from participation in indoor soccer practice or competition.
4. Player and Parent hereby assume any and all risks and hazard of injury to Player in the course of a AK CLEVELAND ELITE FC
event, including, but not limited to injury caused by physical contact with other participants, physical exertion, or playing
conditions, including field conditions.
5. Parent and Player on behalf of themselves and their assignees release and hold harmless AK CLEVELAND ELITE FC from any
and all liability and expense ,including litigation costs and attorney fees arising out of any and all claims which Parent and/or
Player may have, Parent and Player hereby waive all claims they may have against AK CLEVELAND ELITE FC, or any of its
directors, officers, officials, employees, coaches, representatives, and agents, as a result of the Parent/Player participation in a
AK CLEVELAND ELITE FC Event (including any transportation to and from the event) and including any facilities provided directly
or indirectly by AK CLEVELAND ELITE FC.
6. Parents and Players shall hold harmless and indemnify AK CLEVELAND ELITE FC and AK CLEVELAND ELITE FC affiliates from any
and all claims, liabilities, and expenses arising out of claims brought by third parties in connection with any acts or omissions of
the Player and/or any parent, guardian, sibling or other relative of the Player who attends and participates in a AK CLEVELAND
ELITE FC.
7. Parent and Player acknowledge that this document shall be operative as to any AK CLEVELAND ELITE FC Event scheduled as
soccer now or in the future and for which Player is participating regardless of team, participants, facility or location.
8. Player and Parent hereby grants the unrestricted right and permission, free from approval, review or cost, to photograph,
record or otherwise capture the players likeness in all media, now or hereafter known, including, but not limited to pictures ,
video, and Drone footage to copyright the same in its own name, and which may be included in whole or in part for any
commercial or promotional use of at its discretion.
9. This Agreement shall be governed by the laws of the State of Ohio.
10. Player and Parent acknowledge that they have the option of not applying for Player participation in AK CLEVELAND ELITE FC,
and of Player not participating in AK CLEVELAND ELITE FC Events, if the terms of the foregoing are not acceptable.
11. PARENT AND PLAYER HAVE READ THE TERMS OF THIS AGREEMENT AND THEY UNDERSTAND AND FULLY AGREE TO THE
SAME AS BINDINGUPON THEM, THEIR HEIRS AND PERSONAL REPRESENTATIVES.

AK CLEVELAND ELITE FC 2016-2017 PARTICIPATION REQUIREMENTS
Participation in AK CLEVELAND ELITE FC
Any accepted players joining AK CLEVELAND ELITE FC understand they are making a full year commitment to participate. This full year
commitment includes fall, winter and spring sessions or in the case of players in high school this includes winter and spring sessions. Players
may pay all annual fees in one lump sum at the commencement of their first session or players may choose to pay the annual fees by making
monthly payments. There will be a non-refundable deposit of $300 to be deducted from the overall annual fees on return of the necessary
forms for teams participating in MRL, STATE CUP AND GENEVA SCHEDULE ,those teams not participating in MRL or GENEVA will pay $225 to be
deducted from the total annual fees. All payments are due at the start of each season or on a monthly basis as set out above, payment for
showcases, tournaments not included above must be paid 7 days in advance of the said event. Players failing to make payments on time will
not be permitted to participate in training or game play until such time that full payment is made. Roster spots will be held for all players that
make a full year commitment and pay all fees on time.
Registration into AK CLEVELAND ELITE FC
All accepted players into AK CLEVELAND ELITE FC are required to complete all registration steps prior to attending a training session or game.
Registration steps include the following:

Complete registration and make full payment arrangements.

Produce copy of birth certificate, player picture and complete waiver registration.

Full payment must be made prior to participation. There are no refunds.

Order official AK CLEVELAND ELITE FC game uniform kit and official training uniform
Game and Practice Uniforms
All official AK CLEVELAND ELITE FC uniforms are custom manufactured . The club will attempt to keep inventory of replacement training jerseys
and replacement socks for player purchase. However, it is strongly recommended that players purchase multiple sets as players will routinely
train and participate in games.
All players who register to play up to and including the 20th July 2017 will receive the full AK CLEVELAND ELITE FC
uniform kit and the AK CLEVELAND ELITE FC training kit FREE OF CHARGE. This kit should be worn to all AK
CLEVELAND ELITE FC games and practices.





Players are not permitted to modify their game uniform in any way.
Players are not permitted to modify their training uniforms in any way.
Players are not permitted to wear training uniforms displaying their name or player number.
Players are not to make substitutes to the official game or official training uniform.
AK CLEVELAND ELITE FC must pre-approve all selected player numbers for game jerseys. Player numbers for teams/age groups must
each be unique.
Club and Player Fundraisers
Throughout the year AK CLEVELAND ELITE will offer a variety of fundraising opportunities. These fundraising opportunities are in place to
benefit both AK CLEVELAND ELITE FC and act as an opportunity for players to raise funds for the purpose of offsetting their registration fees,
overseas tour fees, tournament fees. Players are required to participate accordingly.
Travel Policy for Tournaments, Showcases and League Games




Players are expected to follow all AK CLEVELAND ELITE FC club travel policies.
Players are expected to stay in team assigned hotel.
Players are required to attend all team meetings. Players should always anticipate that coaches will hold daily team meetings while
travelling.
Players are required to attend all team meals. Any required attendance team meals will be communicated in advance.
Player code of conduct

Players will at all times conduct themselves in accordance with AK CLEVELAND ELITE FC player code of conduct policies.

Disrespect to coaches, teammates, opponents or referees by players will not be tolerated.
Obtain plenty of rest and follow good nutritional practices in preparing for competition.
Parent Section
Parents are asked to familiarize themselves with all club policies. A few highlights include the
Following:
Maintain a respectful distance during all training sessions.
No coaching from the sidelines – ever. This includes all training sessions and all games.




Do your best to have players arrive at the assigned times for all training sessions and for all
games.
Make sure players wear only the official game and training uniforms to all events.
Refer to session communication for a more detailed account of club policies.
