This County of Alameda, General Services Agency (GSA), RFP Addendum has been
electronically issued to potential bidders via e-mail. E-mail addresses used are those in the

Providers will be utilized on an as needed basis, but will not be guaranteed an equal amount of
work amongst other provider. Services requested and assigned will be based on the final
evaluation score. The highest ranked provider will have priority in receiving ergonomic evaluation
requests. If the highest ranked provider is not available, the second highest ranked provider will
then be assigned the ergonomic evaluation requests

Description of the Proposed Services: Bid response shall include a description of the terms and
conditions of services to be provided during the contract term. including response times. The
description shall contain a basis of estimate for services including its scheduled start and
completion dates, the number of Bidder’s and County personnel involved, and the number of hours
scheduled for such personnel. Finally, The description must: (1) specify how the services in the bid
response will meet or exceed the requirements of the County; (2) explain any special resources,
procedures or approaches that make the services of Bidder particularly advantageous to the
County; and (3) identify any limitations or restrictions of Bidder in providing the services that the
County should be aware of in evaluating its Response to this RFP.

Page 5 of the RFP, Section C. Bidder Qualifications, Item 4 states:
“Bidder shall possess a current State of California license in Physical Therapy or
Occupational Therapy.”

Will the County consider accepting certified physical therapy assistants or certified
occupational therapy assistants as well, if they meet the five-year experience and possess a
current nationally recognized certification in ergonomics?
No. The County will not accept certified physical therapy assistants or certified
occupational therapy assistants.

“Schedule and perform the ergonomic evaluation within 10 business days of receiving the
referral.”

Will the County accept “attempt to” schedule and perform the ergonomic evaluation within
10 business days of receiving the referral?
The Contractor shall attempt to schedule and perform the ergonomic evaluation within
10 business days of receiving the referral. The County recognizes the challenges the
Contractor may experience if they cannot reach the client department to confirm an
appointment. The Contractor shall attempt three times (by phone and or email) to reach
the department’s point of contact. If after 30 days the client department has not
returned the Contractor’s phone call or email, the Contractor should email
[email protected] to inform the County that three attempts to schedule and perform
the ergonomic evaluation were made to no avail.

Can the County explain what happens if the Contractor is available within 10 days of
receiving a referral but the employee is not?

Page 6 of the RFP, Section D. Specific Requirements, Item 1 (d) states:
“Include the employee’s supervisor in the ergonomic evaluation or confirm the employee’s
job duties with the employee’s supervisor before submitting the written report.”

Will the County allow Supervisors to review the evaluation report after the assessment is
completed to determine whether the job duties described are correct to not delay the
evaluation process?
Yes. When an ergonomic evaluation request is made by a County department,
information includes the employee’s and employee’s supervisor’s names and email

addresses. After the request is referred to the Contractor from Risk Management Unit
and the evaluator completes the evaluation, the written report needs to be emailed to
the employee and the employee’s supervisor. It is the supervisor’s responsibility to: (1)
review the report; (2) ensure job duties as described by the employee to the evaluator
are accurately captured; and (3) ensure equipment recommended aligns with the
employee’s actual job duties.

Can the County provide the job title on the evaluation request form so the Contractor can
look up the job specifications on the County’s online employment website?
No. The employee provides their job title at the time of evaluation. The County does not
recommend that the Contractor use the County’s online employment website to research
job descriptions and duties. The employee is the best resource to provide this
information.

appropriate, for ergonomic equipment purchases.”

Will the County provide a list of approved vendors for ergonomic equipment to be included
in the report, and can the Contractor assume the purchasing agent will be able to find this
information on their own?
Yes. The County can provide a list of approved vendors for ergonomic equipment.
Contractor will work with Risk Management Unit to identify the vendors that provide
specific equipment.
Can the County’s Risk Management Unit provide training or inform other County agencies
and departments ergonomic assessment procedures such as finding approved ergonomic
vendors and order statuses?
Yes. Risk Management Unit will work with other County agencies and departments to
provide them a better understanding of the ergonomic assessment procedures.

“Contractor shall schedule a follow up appointment to ensure the recommended equipment
is properly installed and being used appropriately.”

Will the Contractor be required to physically follow-up on all appointments, or will an email
or phone call be sufficient?
Contractor will be required to make a follow-up phone call and determine whether a
physical follow-up is needed to adjust ergonomic equipment after it is installed. If the
employee requests an in-person follow-up appointment, the Contractor is required to

schedule and complete a follow-up appointment within 10 business days after all
equipment is received and installed.

Once ergonomic item(s) arrive, can the employee initiate a follow-up appointment with the
Contractor? If yes, can the Contractor assume this time is billable?
Yes. The employee can initiate a follow-up appointment with the Contractor after the
ergonomic items have arrived and are installed by the equipment vendor, County General
Services Agency, County Information Technology Department, or the employee’s
department. The Contractor can bill for the ergonomic evaluation follow-up
appointment.
Q10) Page 7 of the RFP, Section D. Specific Requirements, Item 1 (k)(5) states:
“Contractor shall not penalize the County if employees cancel or reschedule their ergonomic
evaluation appointment within 24 hours.”
Will the contractor be able to charge for a “no-show”? If no, can the County advise how
Contractor can recoup time lost when a “no-show” occurs?
A10) The Contractor will not be able to charge for a “no-show.” If the vendor is on-site at the
County department’s location for a scheduled evaluation and the employee is not
available, the County expects the vendor to contact the ergonomic liaison for the
department and check if other employees who are in the queue and in need of an
evaluation are available to meet with the vendor to complete an evaluation. If
evaluation services are completed with a different employee, the vendor may bill for this
time.
Q11) Page 20 of the RFP, Section P. Account Manager/ Support Staff, Item 2 states:
“Contractor shall also provide adequate, competent support staff that shall be able to
service the County during normal working hours, Monday through Friday. Such
representative(s) shall be knowledgeable about the contract, products offered and able to
identify and resolve quickly any issues including but not limited to order and invoicing
problems.”
Will the Contractor be required to hire a full-time administration support staff to meet the
requirement listed above?
A11) The Contractor’s hourly bid should include all costs needed to perform the services
outlined in the RFQ.
Q12) Page 6 of the Exhibit A, Section (c) References, Item (2) states:
“Bidders are to provide a list of three current and three former clients. References must be
satisfactory as deemed solely by County. References should have similar scope, volume, and
requirements to those outlined in these specifications, terms, and conditions.”

If Bidder cannot provide three former references, will the County accept six current
references instead?
A12) Yes. The County will accept six current references if three former references cannot be
provided.
Q13) Can the County define what a current and former reference is?
A13) A current reference is an individual/company that has consistently used the Bidder’s
services within the last three years. A former reference is an individual/company that has
used the Bidder’s services off and on for the last three or more years.
Q14) Will a Doctor of Chiropractic or a Certified Associate Ergonomist meet the qualification of a
current California license in Physical Therapy or Occupational Therapy?
A14) No. A Doctor of Chiropractic or a Certified Associate Ergonomist does not meet the
qualifications of a current California license in Physical Therapy or Occupational Therapy.
Q15) Is the County expecting the Contractor to only make recommendations or is the Contractor
also coordinating purchase and delivery of ergonomic items?
A15) The Contractor is only providing services related to ergonomic evaluations, not
coordinating the procurement process.
Q16) Is the County interested in looking into a more comprehensive solution or solely an
evaluation?
A16) The County is only outsourcing ergonomic evaluation services.
Q17) Can the County provide their current hourly cost for an ergonomic evaluation?
A17) The current hourly rate for an ergonomic evaluation is $90.00 per hour.
