Overview General review of home modifications is to be provided annually (at the same time as review of any Domiciliary Equipment Services (DES) equipment items held by the client) to ensure that the modifications are being used appropriately and safely and do not present a safety risk to client or carer. The “General Equipment, Home mod Review” form will be used by the reviewer to identify any issues with the home modification that may require a referral for a clinical review. The outcome of the review will be documented and referrals completed as indicated. The review of home modifications is required to ensure ongoing safety of home modification use and to manage the risk of any injury to client or carer. The general review is completed annually to identify any problems with the modification or how it is being used by the client. If any problems are identified, this will result in a referral to a clinician to review the client’s use of the modification and to prescribe a replacement modification if DES is unable to repair modification. See procedure “Completing clinical reviews of home modifications” Ongoing repairs, maintenance and replacement of home modifications remains the responsibility of the home owner as agreed to at the initial prescription. Refer to “Terms and Conditions of Installation of Home Modifications”. General reviews of home modifications are completed for current Disability and Domiciliary Care clients who have been provided with home modifications by the Department for Communities and Social Inclusion (DCSI).
