Please submit this form immediately that a player leaves to the CGFL Registrations’ Secretary in person or
via post to: H. Powell, 48 Shelley Avenue, Wincham, Cheshire CW9 6PH. The form will help the League and
Cheshire FA understand why players leave the game and help us understand how many players a team
currently has in its squad at any one time. No individual players details will be shared with anyone other
than the CGFL. The information we share with the FA will disclose numbers and reasons not individuals
details.
