Community-Run Program is committed to
providing local North Miami-Dade and South
Broward children a foundation for lifelong
learning and advancement through exercise
and education. Y.E.S. Camp is proud to work
towards providing ALL Youth the opportunity
to learn and to be creative, and to foster
physical, mental, social, and emotional
development that prepares Our Youth to be
the best they can be in or out of The Game!

(1) To abide by the rules of Y.E.S. CAMP & INDOOR SOCCER CENTRAL, its affiliated organizations, and sponsors.
Recognizing the possibility of physical injury associated with soccer and in consideration for Y.E.S. CAMP accepting the
registrant for its programs and activities (the “Programs”), I hereby release, discharge and/or otherwise indemnify Y.E.S.
CAMP, its affiliated organizations and sponsors, their management, administrators, and associated personnel, including the
owners of fields and facilities utilized for the Programs, against any claim by or on behalf of the registrant as a result of the
registrant’s participation in the Programs and/or being transported to or from the same, which transportation I hereby
authorize. (2) To hereby give my consent for emergency medical care prescribed by a duly licensed Doctor of Medicine or
Doctor of Dentistry. This care may be given under whatever conditions are necessary to preserve the life, limb or well-being
of me. (3) To hereby give my consent to INDOOR SOCCER CENTRAL to take photographs, video recordings, and/or
sound recordings of the above named player in documenting the activities of Y.E.S. CAMP’s programs. I grant INDOOR
SOCCER CENTRAL permission to use the negatives, prints, motion pictures, video/audio tapings, or any other reproduction
of the same for INDOOR SOCCER CENTRAL’s educational and promotional purposes in manuals, on flyers, on the world
wide web, or in other publications.

By registering for and participating in Y.E.S. CAMP’s Programs, you evidence your agreement to the foregoing Code of
Ethics. Failure to comply with this Code of Ethics may result in the suspension of your privilege to participate in Y.E.S.
CAMP events.
