GFWHL provides quality hockey opportunities for all interested adult skaters with emphasis on skill
development, camaraderie, friendship, fitness, and FUN!! All GFWHL members must be at least 18
years of age. (Under 18 will be considered on an individual / limited basis)

I understand that GFWHL has the right to terminate a player’s membership without a refund for
due cause (ie: unsportsmanlike conduct or other unacceptable behavior.) League fees will not
be refunded.
Upon registering and participating in this activity, I hereby release Andrea L. Hedges, the Great
Falls Women’s Hockey League, Great Falls Community Ice Foundation, IcePlex, their agents,
and any persons officially connected with their activities from liability or damages to my person
or property arising from participating in or my presence at this activity. Further, I allow any first
aid deemed necessary in case of injury. I am 18 years of age or older.
